摘　　要：目的:探讨连续护理模式在老年糖尿病患者中的应用价值,以及对患者心理状态的影响。方法:2020年6月-2021年6月抽取无锡市人民医院收治的老年糖尿病患者80例,随机分为两组,各40例。观察组应用基于“医院-社区-家庭”的连续护理模式,对照组应用社区常规随访护理模式,比较两组护理效果。结果:观察组护理后疾病相关知识知晓率高于对照组,差异有统计学意义(P<0.05);观察组护理后空腹血糖、餐后2 h血糖水平均低于对照组,差异有统计学意义(P<0.05);观察组护理后汉密尔顿焦虑量表、汉密尔顿抑郁量表评分均低于对照组,差异有统计学意义(P<0.05);观察组护理后自我护理能力评分及生活质量评分均高于对照组,差异有统计学意义(P<0.05)。结论:针对老年糖尿病患者运用连续护理模式,可增强疾病相关知识知晓水平,有效控制血糖水平,并可改善心理状况,提升自我护理能力及生活质量。Objective:To explore the application value of continuous nursing mode in elderly diabetic patients and its influence on mental state of patients.Methods:From June 2020 to June 2021,80 elderly diabetic patients were selected from Wuxi People's Hospital,they were randomLy divided into the two groups with 40 cases in each group.The observation group applied the continuous nursing mode based on"hospital-community-family".The control group applied the community routine follow-up nursing mode.The nursing effects of the two groups were compared.Results:The awareness rate of disease-related knowledge after nursing in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).The fasting blood glucose and 2 h postprandial blood glucose after nursing in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The Hamilton depression scale and Hamilton anxiety scale scores of the observation group after nursing were lower than those of the control group,the difference was statistically significant(P<0.05).The scores of self-care ability and quality of life of the observation group after nursing were higher than those of the control group,the differences were statistically significant(P<0.05).Conclusion:The use of continuous nursing mode for elderly diabetic patients can enhance the awareness level of disease-related knowledge,effectively control the blood glucose level,improve the mental state,and enhance the self-care ability and quality of life.
