摘  要:目的探讨无创高频通气(NHFV)联合吸入一氧化氮(NO)对呼吸衰竭早产儿氧合功能的影响。方法选取2014年2月至2017年2月在该院就诊的呼吸衰竭早产儿152例,根据治疗方法的不同分为NHFV组和联合治疗组,各76例,NHFV组给予NHFV进行治疗,联合治疗组在NHFV组治疗的基础上采用吸入NO进行治疗。检测治疗前,治疗后第1、2天患儿氧合指数(OI)、血红蛋白50%饱和时的氧分压(P50)、动脉血二氧化碳分压(PaCO_(2))、肺泡气氧分压(PaO_(2))水平,酶联免疫试剂盒检测血管内皮功能水平,比较两组患儿的临床疗效总有效率。结果联合治疗组治疗后第1、2天的OI、P50、PaO_(2)高于NHFV组,PaCO_(2)水平、血管内皮功能指标水平低于NHFV组,差异均有统计学意义(P<0.05);联合治疗组治疗后第1、2天总有效率高于NHFV组,差异有统计学意义(P<0.05)。结论NHFV联合吸入NO对呼吸衰竭早产儿的治疗效果较好,可提高患儿的氧合功能。Objective To study the effect of non-invasive high frequency ventilation(NHFV)combined with nitric oxide(NO)inhalation on oxygenation function of premature infants with respiratory failure.Methods A total of 152 cases of preterm infants with respiratory failure treated in this hospital from February 2014 to February 2017 were selected.According to different treatment methods,the preterm infants were divided into the NHFV group and the combined treatment group,with 76 cases in each group.The NHFV group was treated with NHFV,while the combined treatment group was treated with inhaled NO on the basis of the NHFV group.The levels of oxygenation index(OI),partial pressure of oxygen at 50%hemoglobin saturation(P50),arterial partial carbon dioxide pressure(PaCO_(2))and alveolar oxygen partial pressure(PaO_(2))were detected before treatment and on the first and second day after treatment.The level of vascular endothelial function was detected by the enzyme-linked immunosorbent assay(ELISA)kit,and the total clinical effective rate of preterm infants was calculated.Results The levels of OI,P50 and PaO_(2) in the combined treatment group were higher than those in the NHFV group,and the levels of PaCO_(2) and vascular endothelial function indexes were lower than those in the NHFV group on the first and second day after treatment,with statistical differences(P<0.05).The total effective rate in the combined treatment group was higher than that in the NHFV group on the first and second day after treatment,with statistical difference(P<0.05).Conclusion The NHFV combined with NO inhalation has a good therapeutic effect on the preterm infants with the respiratory failure,which can improve the oxygenation function of infants.
