摘 要:目的探讨头孢他啶联合盐酸溴己新治疗慢性阻塞性肺疾病合并肺部感染的临床疗效,以及对肺功能、炎性因子的影响。方法选取2015年4月至2020年3月包钢集团第三职工医院收治的140例慢性阻塞性肺疾病合并肺部感染患者,按随机数字表法将其分为观察组(70例)与对照组(70例)。两组采取镇咳、平喘、解痉、祛痰等常规治疗。对照组在此基础上采取头孢他啶治疗,观察组在对照组基础上加用盐酸溴己新治疗。评估两组临床治疗效果及安全性,比较两组临床症状消失时间,比较两组治疗前后肺功能和炎性因子水平。结果观察组治疗总有效率高于对照组(P<0.05)。观察组咳痰咳嗽、肺部湿啰音和气短胸闷消失时间均显著短于对照组(P<0.05)。观察组治疗后第1秒用力呼气容积(FEV_(1))、肺活量(FVC)、FVC/FEV_(1)、C-反应蛋白(CRP)、降钙素原(PCT)及肿瘤坏死因子-α(TNF-α)均显著优于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论头孢他啶联合盐酸溴己新治疗慢性阻塞性肺疾病合并肺部感染的疗效较好、安全性较高,可有效降低患者炎性因子水平,改善其肺功能。Objective To explore the clinical efficacy of ceftazidime combined with bromhexine in the treatment of chronic obstructive pulmonary disease(COPD)complicated with pulmonary infection,and the effects of the combination on pulmonary function and inflammatory factors.Methods A total of 140 patients with COPD complicated with pulmonary infection admitted to Baogang Third Hospital of Hongci Group from April 2015 to March 2020 were selected and randomly divided into the observation group(70 cases)and the control group(70 cases)based on the random number table method.The two groups received conventional treatment with antitussive,antiasthmatic,spasmolytic and expectorant.Based on this,the control group was treated with ceftazidime,and the observation group was treated with bromhexine on the basis of the treatment in the control group.The clinical efficacy and safety of the two groups were evaluated.The disappearance time of clinical symptoms was compared between the two groups.The pulmonary function and the level of inflammatory factors were compared between the two groups before and after treatment.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).The disappearance time of expectoration,cough,lung moist rale and shortness of breath and chest tightness in the observation group was significantly shorter than that in the control group(P<0.05).The forced expiratory volume in 1 second(FEV_(1)),forced vital capacity(FVC),FVC/FEV_(1),C-reactive protein(CRP),procalcitonin(PCT)and tumor necrosis factor-α(TNF-α)in the observation group after treatment were significantly superior to those in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Ceftazidine combined with bromhexine is effective and safe in treating COPD complicated with pulmonary infection,which can effectively reduce the level of inflammatory factors and improve the pulmonary function of patients.
