摘  要:为了解青海省不同动物源志贺菌产超广谱β-内酰胺酶(ESBLs)和头孢菌素酶(AmpC)的基因型分布情况,对75株志贺菌进行表型筛选,PCR法确定其ESBLs和AmpC酶基因型。结果:酶表型筛选呈阳性菌株共50株,其中:1株为单产ESBLs阳性菌,42株为单产AmpC酶阳性菌,7株同时产ESBLs和AmpC酶。40株携带AmpC基因(acc、mir-1、dha、fox),未检出mox和cmy-2基因,其中:17株为单基因型,以MIR-1和ACC为主要基因型;5株同时携带3种基因型;18株同时携带2种基因型,以ACC+MIR-1和FOX+MIR-1型为主要基因型。同时,3株检出ESBLs基因(tem、shv、ctx-m、oxa)。在产ESBLs和AmpC酶的3株菌中,1株菌携带acc、dha、ctx-m、tem、shv基因;1株菌携带oxa、tem、fox基因;1株菌携带mir-1和oxa基因。试验表明,临床分离的产ESBLs与AmpC酶志贺菌携带多种耐药基因的比率较高,对指导临床抗菌治疗具有重要意义。To investigate the genotypic distribution of Shigella producing extended spectrum β-lactamases(ESBLs) and AmpC β-lactamase(AmpC from different animals in Qinghai Province, 75 strains of Shigella received phenotypic confirmation test, and their genotypes were determined by PCR. The results showed that a total of 50 strains were phenotypically positive, one of them was ESBLs, 42 of them were AmpC, and seven of them were ESBLs and AmpC.40 of the fifty phenotypic positive strains harbored AmpC genes(acc, mir-1, dha and fox), but mox and cmy-2 genes were not detected. Of these, 17 were of single genotypes with the main genotypes being MIR-1 and ACC. Some strains carried more than two genotypes, five of them carried three genotypes, and 18 carried two genotypes, with ACC and MIR-1 type, FOX and MIR-1 type being the main genotypes. At the same time, three strains were positive for ESBLs genes(tem, shv, ctx-m and oxa). Three strains of Shigella produced ESBLs and AmpC at the same time. One strain carried acc, dha, ctx-m, tem and shv genes, one carried oxa, tem and fox genes, and one carried mir-1 and oxa genes. The carrying rate of resistance genes of the Shigella strains producing ESBLs and AmpC β-lactamase was high. The present results enriched the epidemiological data of ESBLs and AmpC genotypes in the studied area, and were of great significance for detecting enzyme-producing isolates for antibiotic therapy in clinical practice.
