血必净注射液联合生大黄粉治疗中老年脓毒症患者的临床效果

摘　　要：目的探讨血必净注射液联合生大黄粉治疗中老年脓毒症患者的临床效果。方法回顾性选取2020年6月至2021年8月江苏大学附属医院重症医学科收治的92例脓毒症患者作为研究对象,依据住院期间的治疗方案将患者分为对照组(n=46)和观察组(n=46)。对照组采用传统西医治疗,包括抗感染、液体复苏、器官功能支持等常规治疗,观察组在对照组基础上联合应用血必净注射液和生大黄粉。比较两组的血小板计数、中性粒细胞计数、炎症指标、血清D-乳酸(D-Lac)、病情变化及预后效果。比较两组的不良反应发生情况及28 d病死情况。结果治疗后,观察组的血小板计数高于对照组和治疗前,中性粒细胞计数低于对照组和治疗前(P<0.05);对照组治疗前、后的血小板计数比较,差异无统计学意义(P>0.05)。治疗后,两组的CRP、PCT、IL-6、D-Lac水平均较治疗前降低,且观察组低于对照组(P<0.05)。治疗后,两组的APACHEⅡ、SOFA评分均较治疗前降低,且观察组低于对照组(P<0.05)。两组治疗期间均未发生明显不良反应。观察组的28 d病死率为17.39%,显著低于对照组的39.13%,差异具有统计学意义(P<0.05)。结论在传统西医治疗基础上联合应用血必净注射液和生大黄粉治疗中老年脓毒症患者,可减轻炎症反应,改善患者预后,降低死亡率。Objective To investigate the clinical effect of Xuebijing injection combined with raw rhubarb powder in the treatment of the middle-aged and elderly patients with sepsis.Methods Ninety-two patients with sepsis treated in the intensive care unit of affiliated hospital of Jiangsu university from June 2020 to August 2021 were selected as the study objects retrospectively.According to the treatment plan during hospitalization,the patients were divided into control group(n=46)and observation group(n=46).The control group was given traditional western medicine treatment,including routine treatment such as anti-infection,fluid resuscitation and organ function support.The observation group was combined with Xuebijing injection and raw rhubarb powder on the basis of the control group.The platelet count,neutrophil count,inflammatory indexes,serum D-lactic acid(D-Lac),disease change and prognosis were compared between the two groups.The adverse reactions and 28-day mortality were compared between the two groups.Results After treatment,the platelet count of the observation group was higher than that of the control group and before treatment,and the neutrophil count was lower than that of the control group and before treatment(P<0.05);there was no significant difference in platelet count in the control group before and after treatment(P>0.05).After treatment,the levels of CRP,PCT,IL-6 and D-Lac in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).After treatment,APACHEⅡand SOFA scores of the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).No obvious adverse reactions occurred in both groups during treatment.The 28-day mortality in the observation group was 17.39%,which was significantly lower than 39.13%in the control group,and the difference was statistically significant(P<0.05).Conclusion The combined application of Xuebijing injection and raw rhubarb powder on the
