腔内激光联合传统剥脱术治疗下肢静脉曲张的疗效分析

摘　　要：目的探讨腔内激光联合传统剥脱术对下肢静脉曲张患者的手术治疗效果。方法选择2017年1月-2020年6月于本院接受术后治疗的46例下肢静脉曲张患者作为研究对象,根据随机数表法分为观察组和对照组两组,每组各23例。对照组患者采用传统剥脱术治疗,观察组患者采用腔内激光联合传统剥脱术治疗。对两组患者围手术期指标、术后并发症、生活质量以及下肢静脉曲张复发情况进行评价。结果观察组患者术中出血量(23.42±4.02)ml、术后疼痛时间(2.20±0.38)d、下床时间(10.83±1.54)h和住院时间(5.92±0.94)d等围手术期指标均低于对照组[分别为(47.35±6.96)ml、(3.47±0.62)d、(17.91±2.25)h以及(8.64±1.30)d],差异均有统计学意义(P<0.05);观察组术后2个月内并发症发生率(2例,8.70%)低于对照组(8例,34.78%),差异有统计学意义(P<0.05);术后1个月时,观察组患者慢性静脉功能不全生活质量问卷(CIVIQ-20)中体能部分、心理部分、社会部分以及疼痛部分等评分均高于对照组,差异有统计学意义(P<0.05);术后6个月时,两组患者CIVIQ-20评分均较术后1个月时有显著性升高(P<0.05),但四个维度的评分差异均无统计学意义(P>0.05);观察组患者术后6个月内下肢静脉曲张复发率(2例,8.70%)与对照组(3例,13.04%)的差异没有统计学意义(P>0.05)。结论腔内激光联合传统剥脱术可改善下肢静脉患者围手术期指标,降低术后各种并发症的发生风险,减少疾病复发,促进近期和远期生活质量的提高。Objective To explore the surgical treatment effect of intracavitary laser combined with traditional ablation on patients with varicosity of lower extremities.Methods A total of 46 patients with varicosity of lower extremities those received postoperative treatment in our hospital from January 2017 to June 2020 were selected as the study objects,and were divided into observation group(23 cases)and control group(23 cases)according to random number table method.Patients in the control group were treated with traditional ablation,and patients in the observation group were treated with intracavitary laser combined with traditional ablation.Perioperative indicators,postoperative complications,quality of life,and recurrence of varicosity of lower extremities in the two groups were evaluated.Results The perioperative indicators of intraoperative blood loss(23.42±4.02)ml,postoperative pain time(2.20±0.38)d,bedtime(10.83±1.54)h and hospital stay(5.92±0.94)d in the observation group were significantly lower than those of the control group[(47.35±6.96 ml,3.47±0.62 d,(17.91±2.25)h and(8.64±1.30)d,P<0.05].The incidence of complications in the observation group(2 cases,8.70%)within 2 months after surgery was significantly lower than that of the control group(8 cases,34.78%)(P<0.05).At one month after surgery,the scores of chronic venous insufficiency questionnaire(CIVIQ-20)insection of physical,psychological,social and pain in the observation group were significantly higher than those of the control group(P<0.05).At 6 months after surgery,the scores ofCIVIQ-20 of the two groups were significantly higher than those at 1 month after surgery(P<0.05),but the differences in scores of the four dimensions were not statistically significant(P>0.05).There was no statistically significant difference in the recurrence rate of varicosity of lower extremities between the observation group(2 cases,8.70%)and the control group(3 cases,13.04%)within 6 months after surgery(P>0.05).Conclusions The surgical method of intracavitary laser co
