摘 要:目的探究血清钠、氯离子浓度与病毒性脑炎患儿预后的相关性及对预后的影响。方法选择2017年12月-2019年3月在本院治疗的病毒性脑炎患儿112例作为研究对象,根据脑炎严重程度将其分为重症组(43例)和轻症组(69例),比较两组血清钠、氯离子浓度;根据儿童格拉斯哥预后评分量表(CGOS)得分情况将患儿分为预后不良组(35例)和预后较好组(77例),并比较两组血清钠、氯离子浓度;分析血清钠、氯离子浓度与病毒性脑炎患儿预后的相关性;采用多元logistics回归分析影响病毒性脑炎患者预后的危险因素。结果重症组血清钠、氯离子浓度均低于轻症组,差异有统计学意义(P<0.05);预后不良组血清钠、氯离子浓度均低于预后较好组,差异有统计学意义(P<0.05);病毒性脑炎患儿血清钠、氯离子浓度与病情严重程度及预后均呈负相关(P<0.05);病毒性脑炎患儿血清钠、氯离子浓度呈正相关(r=0.298,P=0.001);经多元logistics回归分析得,血清氯离子浓度≤96 mmol/L、血清钠离子浓度≤135 mmol/L是影响病毒性脑炎患者预后的危险因素(P<0.05)。结论病毒性脑炎患儿血清钠、氯离子浓度与疾病严重程度及预后有关,且为导致患儿预后不佳的危险因素。Objective To explore the correlation between serum sodium and chloride ionconcentration and prognosis of children with viral encephalitis and their influence on prognosis.Methods 112 children with viral encephalitis who were treated in our hospital from December 2017 to March 2019 were selected and divided into severe group and mild group according to the severity of encephalitis.Serum sodium and chloride ionconcentrations of the two groups were compared.According to the score of children's Glasgow Outcome Scale(CGOS),the children were divided into poor prognosis group and good prognosis group,and the serum sodium and chloride ionconcentrations of the two groups were compared.The correlation between serum sodium and chloride ionconcentration and prognosis of children with viral encephalitis was analyzed.Multiple logistic regression analysis was used to analyze the risk factors affecting the prognosis of patients with viral encephalitis.Results The serum sodium and chloride ionconcentrations in severe group were lower than those in mild disease group(P<0.05).Serum sodium and chloride ionconcentrations in poor prognosis group were lower than those in good prognosis group(P<0.05).Serum sodium and chloride ionconcentrations in children with viral encephalitis were negatively correlated with disease severity and prognosis(P<0.05).Serum sodium and chloride ionconcentrations in children with viral encephalitis were positively correlated(r=0.298,P=0.001).Multivariate logistic regression analysis showed that serum chloride ionconcentration≤96 mmol/L and serum sodium ionconcentration≤135 mmol/L were risk factors affecting the prognosis of patients with viral encephalitis(P<0.05).Conclusions The concentration of serum sodium and chloride ion in children with viral encephalitis is related to the severity of the disease and prognosis,and is a risk factor for poor prognosis of children.
