摘　　要：目的探讨婴幼儿发育性髋关节异常(DDH)的早期超声筛查临床价值,以期获得DDH良好的预后。方法回顾性分析2018年6月-2020年10月本院收治的3451例42天-6个月龄婴幼儿首次髋关节超声检查资料,按照传统Graf分型法对发育性髋关节发育不良进行分类,并通过超声检查3-7天、1-2个月、2-3个月、3-6个月龄来统计追踪临床干预治疗情况。结果3451个髋关节中首次检查出DDH 45例,超声检出率约为1.30%,其中Ⅱ型25例,Ⅲ型18例,Ⅳ型2例。临床干预治疗后,随访显示DDH在6月龄内发现临床治愈率高(约88.89%),且发现月龄越小,治愈率越高。结论超声检查对6个月内婴儿髋关节是安全有效的检查方法,可以作为早期DDH的筛查方法和评估DDH干预治疗效果的客观依据。Objective To assess the clinical value of early ultrasound screening for diagnosis of infant’s developmental dysplasia of hip joint(DDH), hoping to obtain good prognosis of DDH.Methods A retrospective analysis of ultrasound features of 3451 infants aged 42 days to 6 months those received ultrasound examination in our hospital during June 2018 and October 2020 was carried out. Traditional Graf’s classification method was used to classify DDH. The efficacy of clinical intervention was tracked by ultrasonography at the time point of 3-7 days, 1-2 months, 2-3 months, and 3 to 6 months, respectively. Results Among the 3451 hip joint, 45 cases of DDH were screen out for the first time, and the ultrasonic detection rate was about 1.30%. Which including 25 cases of type Ⅱ, 18 cases of type Ⅲ and 2 cases of type Ⅳ. After clinical intervention, the follow-up showed that the clinical cure rate of DDH was high within 6 months(about 88.89%), and the younger the month, the higher the cure rate. Conclusions Ultrasound is a safe, effective and practical modality for screening of DDH among infants younger than 6 months. It could be used as a screening method for early DDH and an objective basis for evaluating the efficacy of DDH intervention.
