摘　　要：目的分析慢性阻塞性肺疾病急性加重期(AECOPD)患者血液中血小板计数(PLT)和血小板压积(PCT)与肺功能的关系。方法采用临床回顾性分析对收治的18例AECOPD患者的血常规与肺功能相关指标进行检测。测定血液中PLT、PCT及其它血小板参数,并对白细胞计数(WBC)、中性粒细胞计数和嗜酸性粒细胞计数等各项血常规指标进行测定。肺功能指标检测用肺活量(FVC)及其占预计值百分比,第1秒用力呼气容积(FEV1)及其占预计值百分比和1秒率(FEV1/FVC)。并用SPSS21.0软件对各变量之间相关性进行分析。结果 18例AECOPD患者的PLT、PCT与上述测定肺功能指标之间呈正相关(P<0.05);同时,PLT与嗜酸性粒细胞计数及其百分比之间呈正相关(P<0.05)。结论 AECOPD患者的PLT与PCT水平降低,与肺功能受损程度存在较为显著的相关性。同时,PLT与PCT的测定也为判断AECOPD患者肺功能受损程度提供相关数据支持,在临床监测上可对上述参数变化进行重点分析。Objective To study the relationship between platelet counts(PLT),platelet hematocrit(PCT) and pulmonary function in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Retrospective analysis of clinical data of blood routine and pulmonary function in 18 patients with AECOPD admitted from May 2017 to May 2019.Detect PLT,PCT and other platelet parameters in blood, and measure other blood routine indicators such as leukocyte, neutrophil and eosinophil counts;meanwhile, detect the forced vital capacity(FVC) and FVC account for the expected value percentage(FVC% pred),forced expiratory volume in 1 second(FEV1),FEV1 as a percentage of predicted value(FEV1% pred) and FEV1/FVC.And use SPSS21.0 software to analyze the correlation between the variables.Results There were positive correlations between PLT,PCT and FVC,FVC% pred, FEV1 and FEV1% pred in AECOPD patients(P<0.05).PLT was positively correlated with eosinophil counts and eosinophil percentage(P<0.05).Conclusion The reduction of PLT and PCT levels in AECOPD patients has a significant correlation with the degree of pulmonary function impairment, which provides data supports for predicting the pulmonary function of AECOPD patients, and the changes of the above parameters can be analyzed in clinical monitoring.
