摘  要:目的探讨布鲁菌病脊柱炎(BS)的CT,MRI,SPECT,^(18)F-PET/CT表现,以提高其诊断及鉴别诊断水平。方法回顾性分析2015年~2021年经临床和/或病理证实的15例BS患者CT,MRI,SPECT,^(18)F-PET/CT表现特点。结果 BS可侵犯脊柱任何部位,以腰椎最多,边缘性骨质破坏伴有骨质增生硬化,小破坏区无死骨,破坏范围较大时可见死骨,椎周脓肿局限无流注,其内无钙化。^(18)F-PET/CT显像表现为FDG摄取增高。结论 BS的CT和MRI影像学表现具有特征性,边缘性骨质破坏伴有骨质增生硬化,^(18)F-PET/CT表现为FDG高代谢,多模态影像学能为BS的诊断提供影像学依据,具有较高临床价值。Objective To explore CT,MRI,SPECT and ^(18)F-PET/C manifestations of Brucellosis spondylitis, and improve diagnosis and differential diagnosis.Methods Retrospective analysis based on 15 patients who were diagnosed of Brucellosis spondylitis with clinical or pathological evidences from 2015 to 2021.The CT,MRI,SPECT and ^(18)F-PET/CT manifestations features of all subjects were analyzed.Results Brucellosis spondylitis can affect any segment of the spinal column, but generally attacks the lumbar vertebrae.There is marginal damage of the centrum with hyperostosis osteosclerosis.Sequestrum can be present when the range of bone destruction was more extensive.Paraspinal abscess is limited and without tracking phenomenon and internal calcifications.^(18)F-PET/CT imaging shows FDG uptake increase.Conclusion The CT and MRI manifestations of brucellosis spondylitis are characteristic.There is marginal damage of the centrum with hyperostosis osteosclerosis.^(18)F-PET/CT imaging shows FDG uptake increase.The multimodal image findings can provide important imaging basis for the diagnosis of Brucellosis spondylitis.
