切开挂线对口引流术与低位切开高位挂线术治疗高位复杂性肛瘘的效果对比

摘　　要：目的:比较用切开挂线对口引流术与低位切开高位挂线术治疗高位复杂性肛瘘的效果。方法:选取2017年1月至2020年6月期间于瓮安县中医院肛肠皮肤科就诊的82例高位复杂性肛瘘患者作为研究对象。将这些患者分为对照组(n=42)和观察组(n=40)。对对照组患者进行低位切开高位挂线术,对观察组患者进行切开挂线对口引流术。然后比较两组患者的临床疗效。结果:手术后,两组患者手术的总有效率相比,P>0.05;其创面愈合的时间短于对照组患者,P<0.05;其瘢痕的面积小于对照组患者,P<0.05;其视觉模拟评分法(VAS)的评分、Wexner便秘评分系统的评分、并发症的发生率、病情的复发率均低于对照组患者,P<0.05。结论:用切开挂线对口引流术与低位切开高位挂线术治疗高位复杂性肛瘘的效果均较为理想。与用低位切开高位挂线术治疗高位复杂性肛瘘相比,用切开挂线对口引流术治疗高位复杂性肛瘘的安全性更高,患者伤口愈合的速度更快,病情的复发率更低。Objective:To compare the effect of incision and hanging suture for oral drainage and low incision and high hanging suture for the treatment of high complex anal fistula.Methods:From January 2017 to June 2020,82 patients with high complexity anal fistula in The Department of Anorectal Dermatology of Weng‘an Hospital of Traditional Chinese Medicine were selected as the research object.These patients were divided into control group(n=42)and observation group(n=40).Patients in the control group were treated with low incision and high hanging suture,and patients in the observation group were treated with incision and hanging suture and oral drainage.Then the clinical efficacy of the two groups was compared.Results:After operation,the total effective rate of the two groups was compared,P>0.05;The time of wound healing was shorter than that of control group,P<0.05;The area of scar was smaller than that of control group,P<0.05;The visual analogue scale(VAS)score,Wexner constipation score,incidence of complications and recurrence rate were all lower than those of control group,P<0.05.Conclusion:Incision and hanging suture for oral drainage and low incision and high hanging suture for the treatment of high complex anal fistula are ideal.Compared with the treatment of high complex anal fistula with low incision and high hanging suture,the treatment of high complex anal fistula with incision and hanging suture has higher safety,faster wound healing and lower recurrence rate of the disease.
