急性心肌梗死患者血清心肌肌钙蛋白I的水平与其预后的关系

摘　　要：目的:分析急性心肌梗死(AMI)患者血清心肌肌钙蛋白I(cTnI)的水平与其预后的关系。方法:将2020年1月至12月昆明市延安医院收治的110例AMI患者按照血清cTnI水平的不同分为高水平组(血清cTnI的水平≥5 ng/mL,n=55)和低水平组(血清cTnI的水平<5 ng/mL,n=55)。然后对比两组患者血清心脏生化标记物〔肌酸激酶(CK)、肌酸激酶同工酶MB(CK-MB)〕的水平、左室射血分数(LVEF)、心功能分级及住院期间的病死率。结果:1)与高水平组患者相比,低水平组患者血清CK、CK-MB的水平均更低,其LVEF更高,差异有统计学意义(P<0.05)。2)与高水平组患者相比,低水平组患者住院期间的病死率、其中心功能分级为Ⅲ级、Ⅳ级患者的占比均更低,其中心功能分级为Ⅰ级、Ⅱ级患者的占比均更高,差异有统计学意义(P<0.05)。结论:AMI患者血清cTnI的水平与其预后密切相关。Objective:To analyze the relationship between serum cardiac troponin I(cTnI)level and prognosis in patients with acute myocardial infarction(AMI).Methods:A total of 110 AMI patients admitted to Kunming Yan‘an Hospital from January to December 2020 were divided into high level group(serum cTnI level≥5 ng/mL,n=55)and low level group(serum cTnI level<5 ng/mL,n=55)according to different levels of serum cTnI.Then,the levels of serum cardiac biochemical markers(creatine kinase(CK),creatine kinase isoenzyme MB(CK-MB)),left ventricular ejection fraction(LVEF),cardiac function grade and mortality during hospitalization were compared between 2 groups.Results:1)Compared with the high level group,the serum LEVELS of CK and CK-MB in the low level group were lower,and the LVEF was higher,with statistical significance(P<0.05).2)Compared with the high level group,the mortality and the proportion of patients with cardiac function gradeⅲandⅳwere lower in the low level group,and the proportion of patients with cardiac function gradeⅰandⅱwere higher in the low level group,the difference was statistically significant(P<0.05).Conclusion:The level of SERUM cTnI in PATIENTS with AMI is closely related to its prognosis.
