活血生肌方治疗下肢静脉溃疡疗效观察及对创口愈合的影响

摘　　要：目的:观察活血生肌方治疗下肢静脉溃疡(VLU)的临床疗效,并观察对创口愈合的影响。方法:选取北京小汤山医院2018年7月-2020年9月收治的VLU的患者80例,使用随机数字表法随机分为实验组与对照组,每组各40例。对照组接受曲张静脉剥脱术,术毕即对溃疡外敷凡士林纱布包扎。实验组患者进行曲张静脉剥脱术,同时外敷活血生肌纱布包扎。治疗开始前及治疗后进行溃疡面积测定,评估溃疡愈合率及治疗有效率,采用Bates-Jensen伤口评估工具对伤口局部状况进行评估,采用VCSS评分评价静脉临床严重程度。结果:实验组溃疡愈合率、临床总有效率明显高于对照组,两组患者干预前Bates-Jensen伤口总分、VCSS评分比较,差异无统计学意义(t=0.374、-0.622,P>0.05),干预后两组Bates-Jensen伤口总分、VCSS评分低于干预前,且试验组Bates-Jensen伤口总分、VCSS评分低于对照组,差异有统计学意义(t=3.844、8.814,P<0.05)。结论:活血生肌方治疗VLU,可提高患者的溃疡愈合率及治疗有效率,加快溃疡创面的愈合,同时改善伤口局部状况及静脉临床严重程度。Objective:To observe the clinical effect of Huoxue Shengji Recipe in treating venous leg ulcer(VLU)and to explore its effect on wound healing.Methods:80 patients with VLU treated in the hospital from July 2018 to September 2020 were screened and randomly divided into experimental group and control group,with 40 cases in each group.The control group received varicose vein stripping,the ulcer was bandaged with Vaseline gauze after surgery.The experimental group received varicose vein stripping,the ulcer was wrapped with a square gauze of Huoxue Shengji Recipe after surgery.Ulcer area was measured before and after treatment to evaluate ulcer healing rate and treatment efficiency.Bates Jensen wound assessment tool was used to evaluate the local condition of the wound,and VCSS score was used to evaluate the clinical severity of the vein.Results:The ulcer healing rate and clinical total effective rate of the experimental group were significantly higher than those of the control group.There was no statistically significant difference in the total score of Bates Jensen wound and VCSS score between the two groups before intervention(t=0.374,-0.622,P>0.05)After intervention,the total score of Bates Jensen wound and VCSS score of the two groups were lower than before intervention,and the total score of Bates Jensen wound and VCSS score of the experimental group were lower than those of the control group,with statistically significant difference(t=3.844,8.814,P<0.05).Conclusion:Huoxue Shengji Recipe can improve the ulcer healing rate and treatment efficiency of patients with VLU,accelerate the healing of ulcer wound,and improve the local condition of the wound and the clinical severity of venous ulcer.
