摘　　要：目的:了解肇庆市辖区内医疗机构输血科现状及临床输血管理存在的问题,促进科室的整体进步,逐步达到中华人民共和国卫生部《医疗机构临床用血管理办法》(卫生部令第85号)要求。方法:通过查阅医院科室资料和现场调查,对肇庆市辖区内的57间医疗机构设置、临床用血管理等方面进行调研。结果:肇庆市辖区内的57间医疗机构,其中三级医院共4间,三级医院4间均有独立的输血科,二级医院共23间,其余二级医院均由检验科兼并设置。三级医院中,红细胞类和血浆类用量明显高于血小板和冷沉淀凝血因子类。三级医院普遍的用血量明显高于二级医院,三级医院中,红细胞类和血浆类用量明显高于血小板和冷沉淀凝血因子类。2018—2019年,临床输血管理评分上升。结论:肇庆市辖区内的医疗机构输血科正在逐步建设中,有待进一步完善,临床输血管理虽然存在部分问题,如临床输血有效性评估及各规章制度有待加强。总体上,肇庆市辖区内的医疗机构输血科正逐步走向正规,但部分建设环节缺失,存在薄弱环节。Objective:To understand the current situation of blood transfusion department in medical institutions in Zhaoqing and the existing problems in clinical blood transfusion management,in order to promote the overall progress of departments,and gradually to meet the requirements of Management Measures for Clinical Blood Use in Medical Institutions(Ministry of Health Order NO.58)issued by the Ministry of Health of the People’s Republic of China.Methods:By referring to the data of hospital departments and field investigation,the establishment of 57 medical institutions and the management of clinical blood use in Zhaoqing were investigated.Results:There were 57 medical institutions in Zhaoqing,among which there were four tertiary hospitals,which all had independent blood transfusion department,23 secondary hospitals,and the rest of secondary hospitals were merged and set up by clinical laboratory department.In the tertiary hospitals,the amount of red blood cells and plasma was significantly higher than the amount of platelets and cryoprecipitate thrombin subclasses.The amount of blood used in tertiary hospitals was significantly higher than that in secondary hospitals.In tertiary hospitals,the amount of red blood cells and plasma was significantly higher than that of platelets and cryoprecipitate.From 2018 to 2019,the clinical transfusion management score increased.Conclusion:The department of blood transfusion in medical institutions in Zhaoqing is under construction and needs to be further improved.Although there are some problems in clinical blood transfusion management,such as the evaluation of clinical blood transfusion effectiveness and the rules and regulations need to be strengthened.In general,the blood transfusion department of medical institutions in Zhaoqing is gradually moving towards normality,but some construction links are missing and weak.
