预感性悲伤在晚期肺癌患者主要照顾者应对方式与生活质量间的中介效应

摘　　要：目的探讨晚期肺癌患者主要照顾者预感性悲伤与应对方式、生活质量的关系。方法采用便利抽样法,于2019年1月-2020年12月选取郑州市5所三级甲等医院的300例晚期肺癌患者的主要照顾者为研究对象。采用一般资料调查表、预感性悲伤量表、简易应对方式量表、癌症患者照顾者生活质量调查表对其进行调查。结果晚期肺癌患者主要照顾者预感性悲伤总分为(81.37±7.23)分,积极应对得分(16.31±3.69)分,消极应对得分(9.48±2.58)分,生活质量总分(194.43±39.02)分。主要照顾者预感性悲伤得分与积极应对、生活质量均呈负相关(r=-0.36,P<0.01;r=-0.14,P=0.02),与消极应对呈正相关(r=0.30,P<0.01)。预感性悲伤在应对方式和生活质量之间具有部分中介作用。结论临床医护人员在准确识别晚期肺癌患者主要照顾者预感性悲伤的基础上,加强给予其医护支持、心理疏导,帮助其建立积极应对方式,改善其预感性悲伤水平,以便为晚期肺癌患者提供更优质地照护。Objective To explore the relationship of anticipatory grief to the coping style and life quality of primary caregivers of patients with advanced lung cancer.Methods A total of 300 primary caregivers of patients with advanced lung cancer and treated in year 2019 and 2020 were selected from 5 grade-3 class-A hospitals using the convenience sampling method.All patients were investigated using the general information questionnaire,anticipatory grief scale(AGS),simple coping style scale(SCSQ)and caregivers′quality of life(CQOL).Results The total AGS and life quality score,as well as positive and negative coping scores were 81.37±7.23,194.43±39.02,16.31±3.69 and 9.48±2.58.The AGS score was negatively correlated with the scores of the positive coping and life quality(r=-0.36,P<0.01;r=-0.14,P=0.02),but positively correlated to the score of the negative coping(r=0.30,P<0.01).The anticipatory grief had a partial mediating effect on the coping style and life quality.Conclusion Based on the accurate identification of the anticipatory grief of the primary caregivers of advanced lung cancer patients,the clinical medical staff should strengthen the medical support and psychological counseling to help them to establish positive coping style and provide better care for patients.
