摘　　要：目的 探讨雷公藤多苷联合贝前列素钠治疗对血栓闭塞性脉管炎患者临床疗效、视觉模拟评分法(VAS )及炎性因子的影响。 方法 回顾性分析本院2018年7月~2020年7月收治的血栓闭塞性脉管炎患者70例,根据不同药物方案治疗分为A组(34例,贝前列素钠治疗)与B组(36例,雷公藤多苷联合贝前列素钠治疗),比较两组临床疗效、VAS评分、炎性因子水平、踝肱指数(ABI)、最大步行距离及不良反应。 结果 B组治疗后总有效率显著高于A组(P<0.05)。与治疗前比较,治疗后两组患者麻木、冷感及静息痛VAS评分、肿瘤坏死因子-α(TNF-α) 、白介素-6(IL-6)及 免疫球蛋白(Ig)G均较治疗前降低,且B组患者上述VAS评分、炎性因子TNF-α、IgG及IL-6水平显著低于A组(均P<0.05)。治疗后,两组患者ABI、最大步行距离及生活质量评分则明显较治疗前升高,以B组上升更为显著(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。 结论 雷公藤多苷联合贝前列素钠治疗血栓闭塞性脉管炎患者疗效确切,可有效缓解患者疼痛,抑制体内炎症反应,且不会增加药物不良反应,安全性良好。Objective To analyze the effects of tripterygium wilfordii polyglycosides combined with beraprost sodium in the treatment of patients with thromboangiitis obliterans on clinical efficacy, visual analogy score (VAS) and inflammatory factors. Methods The clinical data of 70 patients with thromboangiitis obliterans admitted to our hospital from July 2018 to July 2020 were retrospectively analyzed. Finally, 70 patients were enrolled and divided into group A (34 cases, beraprost sodium) and group B (36 cases, tripterygium wilfordii polyglycosides combined with beraprost sodium) according to different drug doses. The clinical efficacy, VAS score, inflammatory factor level, ankle brachial index (ABI), maximum walking distance and adverse reactions were compared between the two groups. Results The total effective rate in group A was 73.53% after treatment, and the total effective rate in group B was 91.67%, which was significantly higher than that in group A (P<0.05). Numbness, cold sensation and resting pain VAS score, tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) and immunoglobulin (Ig) in the two groups after treatment were lower than those before treatment, and the above-mentioned VAS score, inflammatory factors TNF-α, IgG and IL-6 levels in group B all were significantly lower than those in group A (P<0.05). After treatment, the ABI and maximum walking distance, and quality of life of the two groups were significantly higher than those before treatment, and the increase in group B was more significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Tripterygium wilfordii polyglycosides combined with beraprost sodium has a definite effect in the treatment of patients with thromboangiitis obliterans. It can effectively relieve pain in patients and inhibit inflammatory reactions in the body without increasing adverse drug reactions and has good safety.
