带线锚钉联合骨隧道缝合治疗髌骨下极骨折的临床疗效

摘　　要：目的探讨对髌骨下极骨折患者行带线锚钉联合骨隧道缝合治疗的临床效果。方法45例髌骨下极骨折患者,按照随机分组法分为对照组(22例)和试验组(23例)。对照组采用常规钢丝张力带固定治疗,试验组采用带线锚钉联合骨隧道缝合治疗。比较两组患者手术时间、骨折愈合时间、术后6个月B?stman髌骨骨折功能评分、膝关节活动度、术后并发症发生情况(固定物松动伴骨折移位、感染、骨延迟愈合)。结果治疗后,试验组手术时间(39.98±2.11)min、骨折愈合时间(68.98±10.39)d均短于对照组的(45.12±3.55)min、(78.77±13.45)d,术后6个月Bostman髌骨骨折功能评分(27.98±2.11)分高于对照组的(24.12±3.80)分,膝关节活动度(125.29±4.49)°大于对照组的(118.00±3.23)°,差异均具有统计学意义(P<0.05)。治疗后,试验组术后并发症发生率4.35%低于对照组的27.27%,差异具有统计学意义(P<0.05)。结论对髌骨下极骨折患者行带线锚钉联合骨隧道缝合治疗临床效果更显著,围术期指标更佳,并发症更少,值得在临床中推广及应用。Objective To discuss the clinical effect of suture-anchor combined with transosseous suture in the treatment of fractures of the lower pole of the patella.Methods A total of 45 patients with fractures of the lower pole of the patella were randomly divided into control group(22 cases)and test group(23 cases).The control group was treated with conventional wire tension band fixation,and the test group was treated with suture-anchor and transosseous suture.Both groups were compared in terms of operation time,fracture healing time,Böstman patellar fracture functional score at 6 months postoperatively,knee range of motion,postoperative complications(loosening of fixation with fracture displacement,infection,delayed union).Results After treatment,the operation time(39.98±2.11)min and fracture healing time(68.98±10.39)d of the test group were shorter than(45.12±3.55)min and(78.77±13.45)d of the control group;the Böstman patellar fracture functional score at 6 months postoperatively(27.98±2.11)points of the test group was higher than(24.12±3.80)points of the control group;the knee range of motion(125.29±4.49)°of the test group was greater than(118.00±3.23)°of the control group.All the differences were statistically significant(P<0.05).After treatment,the postoperative complication rates 4.35%in the experimental group was lower than 27.27%in the control group,and the difference was statistically significant(P<0.05).Conclusion Suture-anchor in combination with transosseous suture has significant clinical effect on patients with fractures of the lower pole of the patella,and it has better perioperative indicators,and fewer complications,which is worth promoting and applying in clinical practice.
