右美托咪定不同给药方式对腹腔镜子宫肌瘤剔除术麻醉效果的影响分析

摘　　要：目的分析右美托咪定不同给药方式对腹腔镜子宫肌瘤剔除术麻醉效果的影响。方法60例腹腔镜宫肌瘤剔除术患者,随机分为对照组和试验组,各30例。对照组给予持续泵注右美托咪定,试验组术前15 min给予单次缓慢静脉滴注右美托咪定。比较两组患者机械痛阈值评分、苏醒期躁动评分及拔管、恢复定向力、麻醉恢复、应答及生命指征恢复稳定时间。结果试验组术后4 h机械痛阈值评分(8.66±0.31)分、8 h机械痛阈值评分(8.33±0.12)分及苏醒期躁动评分(1.80±0.18)分均低于对照组的(12.51±0.39)、(13.27±0.28)、(2.50±0.21)分,差异有统计学意义(P<0.05)。试验组患者拔管时间(14.60±1.72)min、恢复定向力时间(16.82±1.13)min、生命指征恢复稳定时间(20.82±1.11)min、应答时间(17.80±3.64)min及麻醉恢复时间(20.34±5.09)min均短于对照组的(23.21±1.77)、(24.25±1.11)、(26.21±1.12)、(24.34±3.12)、(28.90±5.08)min,差异有统计学意义(P<0.05)。结论对腹腔镜子宫肌瘤剔除术患者给予右美托咪定单次静脉滴注方式给药麻醉效果显著,可有效缩短麻醉恢复时间,值得推广。Objective To analyze the effect of different administration methods of dexmedetomidine on the anesthetic effect of laparoscopic myomectomy.Methods A total 60 patients undergoing laparoscopic myomectomy were randomly divided into control group and test group,with 30 cases in each group.The control group received continuous pump infusion of dexmedetomidine,and the test group received single intravenous infusion of dexmedetomidine 15 min before operation.Both groups were compared in terms of mechanical pain threshold score,agitation score during recovery,extubation time,directional force recovery time,anesthesia recovery time,response time,and vital indicators stabilization time.Results The mechanical pain threshold score at 4 and 8 h after operation,and agitation score during recovery of the test group were(8.66±0.31),(8.33±0.12),and(1.80±0.18)points,which were lower than(12.51±0.39),(13.27±0.28),and(2.50±0.21)points of the control group,and the difference was statistically significant(P<0.05).In the test group,the extubation time was(14.60±1.72)min,the directional force recovery time was(16.82±1.13)min,the vital indicators stabilization time was(20.82±1.11)min,the response time was(17.80±3.64)min,and the anesthesia recovery time was(20.34±5.09)min,which were shorter than(23.21±1.77),(24.25±1.11),(26.21±1.12),(24.34±3.12),and(28.90±5.08)min,and the difference was statistically significant(P<0.05).Conclusion Single intravenous infusion of dexmedetomidine in patients undergoing laparoscopic myomectomy is effective and can effectively shorten the anesthesi recovery time,which is worth promoting.
