阿法骨化醇与吡格列酮治疗多囊卵巢综合征的临床效果

摘　　要：目的分析阿法骨化醇与吡格列酮治疗多囊卵巢综合征的临床效果,及其对胰岛素抵抗、性激素水平的影响。方法90例多囊卵巢综合征患者,随机分为观察组与参照组,各45例。给予观察组阿法骨化醇联合吡格列酮治疗,给予参照组吡格列酮单一治疗。比较两组治疗后的胰岛素抵抗相关指标[空腹胰岛素(FINS)、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、胰岛素抵抗指数(HOMAIR)、胰岛β细胞功能指数(HOMA-β)]及性激素[雌二醇(E_(2))、睾酮(T)、黄体生成素(LH)、游离雄激素指数(FAI)、促卵泡生成激素(FSH)、LH/FSH]水平。结果治疗后,观察组FINS(7.36±1.25)mU/L、FPG(4.20±0.31)mmol/L、2 h PG(6.28±1.01)mmol/L、HOMA-IR(1.42±0.26),均低于参照组的(10.65±1.43)mU/L、(5.11±0.36)mmol/L、(7.93±1.25)mmol/L、(2.00±0.51),差异有统计学意义(t=11.620、12.849、6.888、6.797,P<0.05)。观察组HOMA-β(286.33±21.14)高于参照组的(263.55±20.64),差异有统计学意义(t=5.172,P<0.05)。治疗后,观察组E_(2)、T、LH、FAI、FSH、LH/FSH均低于参照组,差异有统计学意义(P<0.05)。结论阿法骨化醇与吡格列酮的联合应用,可以促进多囊卵巢综合征患者胰岛素抵抗以及性激素水平的有效改善,治疗效果明显,有利于改善患者预后。Objective To analyze the clinical effect of alfacalcidol combined with pioglitazone on polycystic ovary syndrome,and its influence on insulin resistance and sex hormone levels.Methods A total of 90 patients with polycystic ovary syndrome were randomly divided into observation group and reference group,with 45 cases in each group.The observation group was treated with alfacalcidol and pioglitazone,and the reference group was treated with pioglitazone alone.Both groups were compared in terms of related indicators of insulin resistance[fasting insulin(FINS),fasting plasma glucose(FPG),2 h postprandial glucose(2 h PG),homeostasis model assessment of insulin resistance(HOMA-IR),homeostasis model assessment ofβcell function(HOMA-β)]and sex hormones[estradiol(E_(2)),testosterone(T),luteinizing hormone(LH),free androgen index(FAI),follicle stimulating hormone(FSH),LH/FSH]levels after treatment.Results After treatment,the FINS(7.36±1.25)mU/L,FPG(4.20±0.31)mmol/L,2 h PG(6.28±1.01)mmol/L,and HOMA-IR(1.42±0.26)of the observation group were all lower than(10.65±1.43)mU/L,(5.11±0.36)mmol/L,(7.93±1.25)mmol/L,and(2.00±0.51)of the reference group,and the difference was statistically significant(t=11.620,12.849,6.888,6.797;P<0.05).The HOMA-β(286.33±21.14)of the observation group was higher than(263.55±20.64)of the reference group,and the difference was statistically significant(t=5.172,P<0.05).After treatment,the E_(2),T,LH,FAI,FSH,LH/FSH of the observation group were lower than those of the reference group,and the difference was statistically significant(P<0.05).Conclusion Alfacalcidol combined with pioglitazone can promote the improvement of insulin resistance and sex hormone levels in patients with polycystic ovary syndrome,and the treatment effect is obvious,which is conducive to improving the prognosis of patients.
