子宫缝合术与欣母沛治疗子宫收缩乏力性产后出血患者的疗效分析

摘　　要：目的分析子宫缝合术与卡前列素氨丁三醇注射液(商品名:欣母沛)治疗子宫收缩乏力性产后出血患者的疗效。方法200例子宫收缩乏力性产后出血患者,根据采用的治疗方法不同分为参照组与研究组,各100例。参照组采用子宫缝合术治疗,研究组采用子宫缝合术联合欣母沛治疗。比较两组产后2 h出血量、产后24 h出血量、血红蛋白水平、血性恶露时间、月经复潮时间、治疗前后雌二醇、促卵泡激素水平。结果研究组产后2 h出血量、产后24 h出血量低于参照组,血红蛋白水平高于参照组,差异均有统计学意义(P<0.05)。研究组血性恶露时间和月经复潮时间均短于参照组,差异有统计学意义(P<0.05)。治疗后,两组雌二醇、促卵泡激素水平均较治疗前改善,且研究组雌二醇(45.69±2.85)pmol/L高于参照组的(24.92±3.41)pmol/L,促卵泡激素(33.22±2.68)IU/L低于参照组的(66.58±3.29)IU/L,差异有统计学意义(P<0.05)。结论子宫缝合术联合欣母沛治疗不仅减少子宫收缩乏力性产后出血患者产后出血量,还缩短其月经复潮时间和血性恶露时间,减轻对血红蛋白和卵巢功能的影响。Objective To analyze the efficacy of uterine suture and carboprost tromethamine injection(trade name:Hemabate)in the treatment of postpartum hemorrhage due to uterine atony.Methods A total of 200 patients with postpartum hemorrhage due to uterine atony were divided into reference group and research group according to different treatment methods,with 100 cases in each group.The reference group was treated with uterine suture,and the research group was treated with uterine suture combined with Hemabate.Both groups were compared in terms of 2 h postpartum blood loss,24 h postpartum blood loss,hemoglobin level,bloody lochia time,menstruation recovery time,estradiol and follicle stimulating hormone levels.Results The 2 h postpartum blood loss,24 h postpartum blood loss of the research group were lower than those of the reference group,and the hemoglobin level was higher than that of the reference group.All the differences were statistically significant(P<0.05).The bloody lochia time and menstrual recovery time in the research group were shorter than those in the reference group,and the difference was statistically significant(P<0.05).After treatment,the levels of estradiol and follicle stimulating hormone in the two groups were improved compared to those before treatment;the estradiol(45.69±2.85)pmol/L of the research group was higher than(24.92±3.41)pmol/L of the reference group,and follicle stimulating hormone(33.22±2.68)IU/L was lower than(66.58±3.29)IU/L of the reference group;all the difference was statistically significant(P<0.05).Conclusion Uterine suture combined with Hemabate can not only reduce the postpartum blood loss,but also shorten the menstruation recovery time and bloody lochia time,and reduce the impact on hemoglobin and ovarian function.
