董氏奇穴结合耳尖放血对肝郁化火型失眠患者睡眠质量及血清5-HT含量的影响

摘　　要：目的:探讨董氏奇穴结合耳尖放血对肝郁化火型失眠患者的睡眠质量及血清中5-羟色胺(5-HT)含量的影响。方法:将100例肝郁化火型失眠患者随机分为观察组和对照组,每组50例。观察组给予董氏奇穴结合耳尖放血治疗,对照组给予艾司唑仑片治疗,2组均以2周为1个疗程,治疗2个疗程,并随访4周。比较2组患者治疗前后及随访4周的匹兹堡睡眠质量指数量表(PSQI)评分变化及血清中5-HT含量的变化。结果:2组治疗后PSQI评分均较治疗前显著降低(P<0.05),且观察组治疗后的PSQI评分显著低于对照组(P<0.05),随访4周时观察组PSQI评分显著低于对照组(P<0.05),其中观察组的睡眠质量、睡眠效率及日间功能评分较对照组有显著降低(P<0.05),但在入睡时间与睡眠时间2组比较差异无统计学意义(P>0.05);2组治疗后血清中5-HT含量均较治疗前显著升高(P<0.05),但2组治疗后血清中5-HT含量比较差异无统计学意义(P>0.05),随访4周时观察组血清中5-HT含量明显高于对照组(P<0.05)。结论:董氏奇穴结合耳尖放血治疗能够显著改善肝郁化火型失眠患者的睡眠状况,尤其是睡眠质量、睡眠效率及日间功能方面,其机制可能与提升血清中5-HT含量有关。Objective:To investigate the effects of Dong′s Extra Acupoints combined with ear tip bloodletting on sleep quality and serum 5-HT in insomniac due to liver depression forming fire type.Methods:A total of 100 patients with liver depression forming fire type insomnia were randomly divided into treatment group and control group,with 50 cases in each group.The treatment group received Dong′s Extra Acupoints combined with ear tip bloodletting for 4 weeks,and the control group was treated with Estazolam tablet for same time.The follow-up observation period was 4 weeks.The scores of the Pittsburgh Sleep Quality Index(PSQI),the serum 5-HT in insomniac before and after treatment were observed.Results:The PSQI score of the two groups after treatment was significantly lower than that before treatment(P<0.05),and the PSQI score of the treatment group was significantly lower than that of the control group(P<0.05).At 4 weeks of follow-up,the PSQI score of the treatment group was significantly lower than that of the control group(P<0.05).The sleep quality,sleep efficiency and daytime function score of the treatment group were significantly lower than those of the control group(P<0.05),but there was no significant difference between the two groups in sleep latency and sleep time(P>0.05).The content of serum 5-HT in the two groups after treatment was significantly higher than that before treatment(P<0.05),but there was no significant difference between the two groups after treatment(P>0.05).At 4W follow-up,the content of serum 5-HT in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion:Dong′s Extra Acupoints combined with ear tip bloodletting therapy can remarkably improve the sleep status of patients with liver depression forming fire type insomnia,especially in terms of sleep quality,sleep efficiency and daytime function.The mechanism may be related to the increase of serum 5-HT content.
