摘　　要：目的:建立利拉鲁肽药物利用评价(DUE)标准,评价某三甲医院利拉鲁肽注射液的使用情况。方法:参考利拉鲁肽注射液药品说明书、相关指南和专家共识、专家指导意见等建立利拉鲁肽注射液DUE标准,利用临床合理用药软件检索该院2020年使用利拉鲁肽的所有病例,从中随机抽取100份,评价药物使用的合理性。结果:100份病历中,完全符合利拉鲁肽DUE标准28例(28.00%)。利拉鲁肽的不合理使用主要表现在用药指征和用法用量不合理,其中用药指征不符合标准35例,用法用量不符合标准64例。100份病例中,血糖控制好转97例,体质指数(BMI)下降26例。用药期间发生26例药品不良反应,25例表现为胃肠道反应,1例表现为皮疹,其中1例因无法耐受胃肠道反应而停止使用利拉鲁肽。结论:利拉鲁肽注射液能显著降低患者的血糖水平,但临床在使用过程中存在不合理用药现象,开展DUE工作,可提高药物的合理使用,可规范临床药师的药物点评工作。Objective:To establish a drug use evaluation(DUE)standard for liraglutide injection to evaluate the application of liraglutide injection in a third grade hospital.Methods:The DUE standard of liraglutide injection was established by referring to the drug manuals,relevant guidelines,expert consensus and expert guidance.The clinical rational drug use software was used to retrieve all the cases of liraglutide in a third grade hospital in 2020.One hundred samples were randomly selected to evaluate the rationality of the drug use.Results:Among the 100 medical records,28 cases(28.00%)fully met the DUE standard.The irrational use of liraglutide was mainly manifested in unreasonable indications and dosage including 35 cases of non-standard indications and 64 cases of non-standard dosage.Among the 100 cases,blood glucose control was improved in 97 cases and body mass index(BMI)was decreased in 26 cases.Adverse drug reactions occurred in 26 cases including gastrointestinal reactions in 25 cases and rash in one case,and of which one case stopped taking liraglutide due to intolerance of gastrointestinal reactions.Conclusion:Liraglutide injection can significantly reduce the blood glucose level of patients,however,irrational drug use exists in clinical application.DUE work can improve the rational drug use and standardize the drug comment work of clinical pharmacists.
