妊娠期糖尿病患者血糖控制效果对分娩结局分娩后代谢综合征影响

摘　　要：目的探讨妊娠期糖尿病患者血糖控制效果对母婴健康的影响。方法选取2019年3月—2021年3月于百色市人民医院分娩的220例妊娠期糖尿病患者作为研究对象,患者妊娠期间接受相同的血糖控制指导(包括健康教育、饮食控制、运动锻炼),其中血糖控制效果良好的138例为研究组,血糖控制效果较差的82例为对照组。对比分析2组分娩前代谢性指标、分娩结局和分娩后代谢综合征发生率。结果研究组空腹血糖(FPG)、糖耐量、体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)及不良分娩结局发生率、分娩后代谢综合征发生率分别为(4.08±0.95)mmol/L、(5.83±1.14)mmol/L、(27.61±3.12)kg/m^(2)、(14.57±1.36)kPa、(8.80±1.05)kPa、5.80%、5.07%,低于对照组的(5.98±2.03)mmol/L、(9.72±1.56)mmol/L、(30.89±3.45)kg/m^(2)、(16.69±3.08)kPa、(10.53±2.24)kPa、18.29%、14.63%,差异有统计学意义(P<0.05)。结论增强妊娠期糖尿病患者血糖控制效果,可维持分娩前代谢指标平稳,进而改善分娩结局,减少分娩后代谢综合征的发生,保证母婴安全。Objective To investigate the effect of blood glucose control on maternal and infant health in patients with gestational diabetes mellitus.Methods A total of 220 patients with gestational diabetes who delivered in Baise People's Hospital of Guangxi from March 2019 to March 2021 were selected as the research subjects.The patients received the same blood glucose control instruction(including health education,diet control,exercise and exercise)during pregnancy,and 138 patients with good blood glucose control effect were selected as the study group.The 82 patients with poor glycemic control were the control group.Metabolic indexes before delivery,delivery outcome and incidence of metabolic syndrome after delivery were compared and analyzed between the two groups.Results The incidence of fasting blood glucose(FPG),glucose tolerance,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),adverse delivery outcomes and post-delivery metabolic syndrome in the study group were(4.08±0.95)mmol/L,(5.83±1.14)mmol/L,(27.61±3.12)kg/m^(2),(14.57±1.36)kPa,(8.80±1.05)kPa,5.80%,5.07%,Lower than the control group(5.98±2.03)mmol/L,(9.72±1.56)mmol/L,(30.89±3.45)kg/m^(2),(16.69±3.08)kPa,(10.53±2.24)kPa,18.29%,14.63%,The difference was statistically significant(P<0.05).Conclusion Enhancing the glycemic control effect in patients with gestational diabetes can maintain the stability of metabolic indicators before delivery,improve the outcome of delivery and reduce the occurrence of metabolic syndrome after delivery.In this regard,it is necessary to take active intervention measures to promote the stability of blood glucose level in patients and ensure the safety of mothers and infants.
