摘  要:目的探讨湖北恩施不同硒水平环境老年人群血清硒与骨质疏松相关性及硒暴露情况。方法随机选取2018年6月至2020年5月在恩施州中心医院健康体检的60岁以上来源于富硒足硒缺硒地区老年人各200名,按骨密度(bone mineral density,BMD)测量值及来源地分组,检测血清硒浓度、骨代谢血清学标志物水平及居民饮用水,原产土豆和玉米的硒含量,进行统计分析,计量资料两组间比较采用t检验,多组间比较采用单因素方差分析,率的比较采用卡方检验,进一步两两比较采用SNK—q检验,相关分析采用Spearman相关检验。结果研究对象按骨密度分为正常骨量组(152名)、骨量低下组(242名)和骨质疏松组(206名).三组人群骨密度比较,差异具有统计学意义(F=1.032,P<0.05).骨质疏松组和骨量低下组的血硒含量分别为(78.254±13.256)μg/L、(103.124±12.425)μg/L,均低于正常组的(222.845±15.182)μg/L,差异具有统计学意义(F=22.254,P<0.05)。富硒足硒缺硒地区人群血硒含量为(262.124±13.268)μg/L、(145.987±11.243)μg/L和(94.124±8.103)μg/L,不同硒环境人群血硒含量差异具有统计学意义(F=8.652,P<0.01)。不同硒环境人群骨代谢标志物差异具有统计学意义(P<0.05)。富硒地区饮用水及农作物硒含量高于足硒及缺硒地区。血硒与骨密度有正相关(r值=0.602,P<0.05)。结论恩施地区老年人群血硒与骨代谢相关,不同硒环境地区的饮用水及农作物中硒含量具有差异,有可能为临床上骨质疏松防治提供新思路。Objective To investigate the relationship between serum selenium level and osteoporosis in elderlypopulation in Enshi,Hubei Province.Methods A total of 600 elderly people over 60 years who were admitted to the Central Hospital of Enshi Prefecture for physical examination from June 2018 to May 2020 were randomly selected and divided into groups according to BMD and origin area.Serum selenium、the bone metabolic and biochemical markers and selenium distributions of staple crops and drinking water were determined.The t-test was used for the comparison of continuous data between two groups;one-way analysis of variance was used for comparison of continuous data between multiple groups,and the SNK-q test was used for further comparison between two groups.The Spearman correlation test was used to investigate the correlation between serum selenium and osteoporosis.Results All of elderly population were divided to normal-bone mass group(n=152),low-bone mass group(n=242)and osteoporosis group(n=206).There was a statistically significant difference in BMD of the three groups(F=1.032,P<0.05).The serum selenium in the osteoporosis group and the low-bone mass group was significantly lower than that in the normal-bone mass group[(78.254±13.256)μg/L、(103.124±12.425)μg/L vs.(222.845±15.182)μg/L](F=22.254,P<0.05).The contents of the serum selenium in the poor selenium areas was(94.124±8.103)μg/L with(145.987±11.234)μg/L for the adequate selenium areas and(262.124±13.268)μg/L for the rich selenium areas(F=8.652,P<0.01).There were statistically significant differences in the level of the bone metabolic and biochemical markers of the three groups(P<0.05).The selenium levels of drinking water and staple crops in the rich selenium areas were significantly higher than that in the adequate selenium areas and the poor selenium areas.There is a positive correlation between serum selenium and BMD(r=0.602,P<0.05).Conclusion The levels of serum selenium play a great role in bone metabolism.Selenium distributions in drinking wat
