超声心动图对慢型克山病诊断意义

摘　　要：目的以《克山病诊断》(WS/T 210—2011)为指南,对41例慢型克山病超声心动图结果进行分析,与原有标准中的部分内容进行比对,探讨开展超声心动图检查的必要性,进一步丰富诊断内涵,提高诊断的科学性和覆盖范围。方法采用流行病学统计方法,对四川省"克山病病情调查"项目确诊的41例慢型克山病超声心动图诊断结果,与X线胸片诊断、心功能分级结果进行比较,找出内在关联性与差异点,确立心脏超声检查在慢型克山病诊断中的作用。结果将41例慢型克山病患者超声心动图各项检查内容与X线胸片(按照心胸比例确定心脏大小)结果比较,经统计学处理无显著性差异,与心功能分级比较呈现出心功能状态与超声心动图异常检出的各项相关指标有较好的关联性。结论《克山病诊断》中超声心动图异常判定基准,从辅助诊断环节,为克山病诊断提供了更多的循症方法以及可供参照的内容,从而减小了诊断难度,提高了诊断准确性。在诊断过程中有可供参照的意义,对指导治疗,合理用药,调整药物剂型、剂量,以及保障全程用药安全亦有帮助。Objective Guided by the diagnostic standard for Keshan disease(WS/T 210—2011),the echocardiographic results of 41 cases of chronic Keshan disease were analyzed and compared with some contents in the original standard to explore the necessity of echocardiography,to further enrich the diagnostic connotation and improve the scientificity and coverage of diagnosis.Methods Using epidemiological statistical methods,the echocardiographic diagnostic results of 41 cases of chronic Keshan disease diagnosed by the"Keshan disease investigation"project in Sichuan Province were compared with the results of X-ray chest film diagnosis and cardiac function classification,so as to find out the internal correlation and differences,and establish the role of echocardiography in the diagnosis of chronic Keshan disease.Results There was no significant difference between the results of echocardiography and chest X-ray(heart size was determined according to the cardiothoracic ratio)in 41 patients with chronic Keshan disease.Compared with the classification of cardiac function,there was a good correlation between the state of cardiac function and the relevant indexes detected by echocardiography.Conclusion The criteria of ultrasonocardiogram abnormality in the diagnostic criteria of Keshan disease provides more evidence-based methods and reference contents for the diagnosis of Keshan disease from the auxiliary diagnosis link.Thus,the difficulty of diagnosis is reduced and the accuracy of diagnosis is improved.It not only has reference significance in the diagnosis process,but also helps to guide treatment,rational drug use,adjust drug dosage form and dose,and ensure the safety of drug use in the whole process.
