癌痛消方联合TACE治疗原发性肝癌临床疗效及对免疫因子的影响

摘　　要：目的观察癌痛消方联合肝动脉栓塞化疗(TACE)治疗原发性肝癌(PLC)的疗效及对免疫因子的影响。方法将我院收治的114例PLC患者按随机数字表法分为观察组与对照组各57例。对照组采用TACE治疗,观察组采用癌痛消方联合TACE治疗,比较两组临床疗效,治疗前后中医症状得分、免疫因子和肿瘤标志物水平。结果治疗后,观察组客观缓解率高于对照组,观察组疾病控制率高于对照组(P<0.05)。两组中医症状得分均下降,且观察组中医症状均低于对照组(P<0.05)。两组甲胎蛋白(AFP)、糖类抗原724(CA724)、CA242水平均下降,观察组患者AFP、CA724、CA242水平均低于对照组(P<0.05)。两组免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、自然杀伤细胞(NK)、CD4^(+)均增加,CD8^(+)均下降;且观察组IgA、IgG、NK、CD4^(+)均高于对照组,CD8^(+)低于对照组(P<0.05)。结论癌痛消方联合TACE治疗PLC患者能有效控制肿瘤进展,提高近期疗效,缓解中医症状,恢复免疫功能。Objective To observe the efficacy of Aitongxiao Decoction and Transcatheter arterial chemoembolization(TACE)in treatment of primary liver cancer(PLC)and its influence on immune factors.Methods A total of 114 patients with PLC were randomly divided into two groups,57 cases each.Control group was treated with TACE,and observation group was treated with Aitongxiao Decoction and TACE.The clinical efficacy,TCM symptom scores,immune factors and tumor marker levels before and after treatment were compared between the two groups.Results After treatment,the objective response rate of observation group was higher than that of control group,and the disease control rate of observation group was higher than that of control group(P<0.05);the scores of TCM symptoms in both groups decreased;the TCM symptoms of observation group were lower than those of control group(P<0.05);the levels of alpha-fetoprotein (AFP),carbohydrate antigen 724(CA724),and CA242 decreased in both groups;the levels of AFP,CA724,and CA242 in observation group were lower than those in control group(P<0.05);immunoglobulin A(IgA),IgG,natural killer cell(NK),and CD4^(+) increased in both groups;CD8^(+) decreased;IgA,IgG,NK,and CD4^(+) in observation group were all higher than those in control group,while CD8^(+) was lower than control group(P<0.05).Conclusion The combination of Aitongxiao Decoction and TACE in treatment of PLC patients could effectively control tumor progression,improve short-term curative effect,relieve TCM symptoms,and restore immune function.
