舒血宁辅治慢性肺心病急性加重期临床疗效及对患者呼吸功能、血管内皮功能的影响

摘　　要：目的观察舒血宁辅治慢性肺心病(PHD)急性加重期临床疗效及对患者呼吸功能、血管内皮功能的影响。方法将我院收治的104例PHD患者随机分为观察组与对照组各52例。对照组参考指南予常规治疗,观察组在对照组的治疗基础上联合舒血宁静脉滴注治疗,均连续治疗14d。评估两组临床疗效,比较两组治疗前后血氧分压(PaO_(2))、血二氧化碳分压(PaCO_(2))、用力肺活量(FVC)、第1s用力呼气容积(FEV1)、FEV1/FVC比值、血清内皮素-1(ET-1)、一氧化氮(NO)、血浆黏度、红细胞比容(HCT)指标,观察两组治疗过程中不良反应。结果观察组总有效率显著高于对照组(P<0.05);治疗后,观察组Pa CO_(2)、ET-1、血浆黏度、HCT指标均低于对照组(P<0.05),PaO_(2)、FVC、FEV1、FEV1/FVC、NO指标均高于对照组(P<0.05);两组治疗期间均无严重不良反应。结论舒血宁辅治PHD急性加重期疗效确切,能改善血管内皮功能及血液黏滞情况,从而恢复患者呼吸功能,且安全性较高。Objective To observe the effects of Shuxuening in the adjuvant treatment of acute exacerbation of chronic pulmonary heart disease(PHD)and its influence on respiratory function and vascular endothelial function.Methods A total of 104 patients with PHD were randomly divided into observation group and control group,52 cases in each group.Control group was given routine treatment with reference to guidelines,while observation group was additionally given intravenous drip of Shuxuening.All were treated continuously for 14d.The clinical curative effect in both groups was evaluated.The partial pressures of blood oxygen(PaO_(2)),carbon dioxide(PaCO_(2)),forced vital capacity(FVC),forced expiratory volume in one second(FEV1),FEV1/FVC,serum endothelin-1(ET-1),nitric oxide(NO),plasma viscosity and hematocrit(HCT)before and after treatment were compared between the two groups.The adverse reactions during treatment in both groups were observed.Results The total response rate in observation group was significantly higher than that in control group(P<0.05).After treatment,PaCO_(2),ET-1,plasma viscosity and HCT were lower in observation group than control group(P<0.05),while PaO_(2),FVC,FEV1,FEV1/FVC and NO were higher in observation group(P<0.05);there were no serious adverse reactions in either group during the treatment.Conclusion Effect of Shuxuetong is significant in the adjuvant treatment of acute exacerbation of chronic PHD,which can improve vascular endothelial function and blood viscosity,and thereby recovering respiratory function,with high safety.
