摘  要:目的通过对比分析评价利用智能手机简化增强现实(AR)技术辅助定位引流术治疗老年幕上脑出血的应用价值。方法回顾性分析221例引流手术治疗的老年幕上脑出血患者(出血量20~40 mL)的临床资料,其中行简化AR技术辅助定位引流术(观察组)108例,行立体定向引流术(对照组)113例。对比两组患者手术时间,置管位置准确率,血肿清除率,术后患肢肌力改善明显率,预后良好率的差异,评价利用智能手机简化AR技术辅助定位引流术治疗老年幕上脑出血的应用价值。结果观察组手术时间平均为(32.8±7.5)min,对照组手术时间平均(97.3±11.4)min,两组比较差异有统计学意义(P<0.05)。观察组置管准确率,血肿清除率,术后肺部感染率,术后下肢静脉血栓发生率,术后患肢肌力改善明显率,预后良好率分别为94.4%,85.4%±7.9%,18.5%,12.0%,76.8%,72.2%;对照组为94.7%,90.1%±6.5%,21.2%,15.0%,82.3%,78.8%;两组比较差异无统计学意义(均P>0.05)。结论利用智能手机简化AR技术辅助定位引流术治疗老年幕上脑出血,与立体定向引流术比较,可明显缩短手术时间,操作过程更简便,无需专业设备辅助,但手术的安全性和可靠性无明显差别。Objective To evaluate the application value of smart phone simplified augmented reality(AR)assisted positioning drainage in the treatment of elderly supratentorial intracerebral hemorrhage.Methods The clinical data of 221 elderly patients with supratentorial intracerebral hemorrhage(bleeding volume 20~40 mL)treated by drainage were analyzed retrospectively,including 108 cases of simplified AR assisted localization drainage(observation group)and 113 cases of stereotactic drainage(control group).The differences in operation time,accuracy of catheter placement,hematoma clearance,postoperative pulmonary infection rate,incidence of postoperative lower extremity venous thrombosis,significant improvement rate of postoperative limb muscle strength and good prognosis between the two groups were compared,and the application value of simplified AR technology assisted localization drainage with smart phone in the treatment of senile supratentorial intracerebral hemorrhage was evaluated.Results The average operation time was(32.8±7.5)min in the observation group and(97.3±11.4)min in the control group.There was significant difference between the two groups(P<0.05).In the observation group,the accuracy of catheterization,the clearance rate of hematoma,the postoperative pulmonary infection rate,the incidence of postoperative lower extremity venous thrombosis,the obvious improvement rate of postoperative limb muscle strength,and the good prognosis rates were 94.4%,85.4%±7.9%,18.5%,12.0%,76.8%and 72.2%.The control group was 94.7%,90.1%±6.5%,21.2%,15.0%,82.3%,78.8%.There was no significant difference between the two groups(all P>0.05).Conclusion Compared with stereotactic drainage,simplified AR technology assisted by smart phone can significantly shorten the operation time,simplify the operation process,and do not need professional equipment,but there is no significant difference in the safety and reliability of the operation.
