纳武利尤单抗致免疫相关性脑炎44例文献分析

摘　　要：目的:分析纳武利尤单抗致免疫相关性脑炎的发生情况、临床特征及治疗与转归,为纳武利尤单抗在临床治疗中出现的免疫相关性脑炎的鉴别及处置提供参考。方法:检索中国知网、维普、万方、PubMed及Elsevier/Science Direct等数据库关于纳武利尤单抗致免疫相关性脑炎的文献报道并进行分析。结果:纳武利尤单抗致免疫相关性脑炎的个案报道共44例,平均发生时间为(59.9±67.3)d,其影像学特征主要为T2/FLAIR高信号,脑电图特征主要为弥漫性慢波。28例患者经停药及对症治疗后痊愈或好转。死亡16例,其中10例与纳武利尤单抗致免疫相关性脑炎有关。结论:纳武利尤单抗致免疫相关性脑炎死亡率较高,在治疗过程中需密切关注患者神经系统症状,鉴别其可能导致的免疫相关性脑炎并及时采取相应的治疗措施。OBJECTIVE To analyze the occurrence,clinical features,treatment and outcome of immune-related encephalitis caused by nivolumab,so as to provide reference for the identification and treatment of immune-related encephalitis in the clinical treatment with nivolumab.METHODS The literature reports on immune-related encephalitis caused by nivolumab were searched and analyzed from databases such as CNKI,VIP,Wanfang,PubMed and Elsevier/Science Direct.RESULTS A total of 44 cases of immune-related encephalitis caused by nivolumab were reported.The average occurrence time was(59.9±67.3)d.The imaging features were mainly T2/FLAIR hyperintensity,and the electroencephalogram features were mainly diffuse slow wave.Twenty-eight patients recovered or improved after stopping the drug and symptomatic treatment.Sixteen patients died,and among them,10 patients were related to immune associated encephalitis caused by nivolumab.CONCLUSION Nivolumab causes a high mortality rate of immune-related encephalitis.It is necessary to pay close attention to the patient’s neurological symptoms during the treatment with nivolumab,identify immune-related encephalitis and take appropriate treatment measures in time.
