摘 要:目的:通过测量正常成人胸椎中三种不同螺钉(椎弓根螺钉TT、经肋横突关节螺钉CU、皮质骨螺钉CBT)钉道的CT断层解剖学参数,为临床应用不同类型螺钉提供解剖学参考数据。方法:选取2017年1月—2018年1月在郴州市第一人民医院行胸椎三维CT检查80例,应用ADW4.6软件测量三种螺钉的最佳长度、最大直径、螺钉的侧倾角与头倾角。结果:所有T1~T12数据分为男女两组,男40例,女40例,年龄18~60岁,平均42.8岁。在胸椎三种固定螺钉中,同一节段水平上CBT螺钉最短,经CU螺钉最长,且三种螺钉长度随着节段的增加而增长;螺钉的最大直径测量以CBT螺钉最细,经CU螺钉最粗;螺钉的侧倾角中TT螺钉与经CU螺钉随胸椎节段的增加而减少,而CBT螺钉的侧倾角呈反抛物线变化。同一种螺钉男、女两组各参数采用两个独立样本的t检验,以P<0.05有统计学意义。结论:本研究测量了成人胸椎CBT、TT及经CU螺钉的CT断层解剖学参数,为临床医师根据临床实际情况使用不同螺钉提供参考。Objective:By measuring the CT sectional anatomical parameters of three different screws(pedicle screw TT,costotransverse joint screw CU,cortical bone screw CBT)in thoracic vertebrae of normal adults,we can provide CT anatomical reference data for clinical application of different types of screws.Methods:From January 2017 to January 2018,80 cases of thoracic vertebrae were examined by 3D CT in Chenzhou No.1 People’s Hospital.The optimal length,maximum diameter,lateral angle and head angle of the three screws were measured by ADW 4.6 software.Results:All T1~T12 data were divided into male and female groups,aged 18~60 years,40 males and 40 females,with an average age of 42.8 years.Among the three types of thoracic spinal screws,the CBT screw was the shortest and the CU screw was the longest through the same segment level,and the length of the three types of screws increased with the increase of the segment.The CBT screw was the thinnest and the CU screw was the thickest.In the roll angle of the screw,the TT screw and the trans-CU screw decreased with the increase of the thoracic spine,while the roll angle of the CBT screw changed in a parabolic manner.Each parameter of the two groups of male and female of the same screw was tested by t test of two independent samples,and P<0.05 was statistically significant.Conclusion:This study measured CT tomographic anatomy parameters of twelve thoracic vertebrae CBT,TT,and CU screws,and provided a reference for clinicians to use different screws according to the actual clinical situation.
