法舒地尔联合ACEI及ARB对老年慢性充血性心力衰竭患者心功能的影响

摘　　要：目的探讨法舒地尔联合血管紧张素转化酶抑制剂(ACEI)及血管紧张素Ⅱ受体阻滞剂(ARB)在老年慢性充血性心力衰竭(CHF)患者中的价值。方法选取2020年2月-2021年2月收治的94例老年CHF患者,按随机数字表法分为两组,每组各47例。对照组给予ACEI与ARB,联合用药组加用法舒地尔,持续连续用药2周。对两组临床疗效、心功能指标、不良反应进行对比。结果联合用药组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗前,两组左室收缩末期内径(LVESD).左室舒张末期内径(LVEDD).左室射血分数(LVEF)相比,差异无统计学意义(P>0.05);治疗2周后,联合用药组LVESD.LVEDD低于对照组,LVEF高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率相当,差异无统计学意义(P>0.05)。结论相较于ACEI与ARB的单用,法舒地尔联合ACEI及ARB更能够有效减轻老年CHF患者症状,改善其心功能,且不会增加不良反应,值得推广应用。0bjective To explore the value of fasudil combined with angiotensin converting enzyme inhibitor(ACEI)and angiotensin receptor blockers(ARB)in elderly patients with chronic congestive heart failure(CHF).Methods 94 elderly CHF patients admitted to our hospital from Felbruary 20201o February 2021 were selected and div ided into Iwo groups according to random number lable method,with 47 rases in each group.The control group was given ACEI and ARB,combination group was addied with suldil fasudil for 2 weeks.Clinical efficacy,eardiae function indexes and adverse reactions were compared belween the Iwo groups.Results The total effective rate of observaion group was higher than control group.the diference was stistically signifrant(P<0.05).Before trealment,there was no signifirant differenrce in left ventricular end-systolic diameter(I.VESD),left ventricular end-diastolie diameter(LV EDD)and left ventricular rjection fraction(LVEF)belween the Iwo groups(P>0.05).Afer 2 weeks of treatment.LV ESD and LV EDD of the ohservation group wer lower than those of the control group,.L.V EF was higher than that of the control group.the dfferenre was stistically signifcant(P<0.05).There was no significant iferenre in the ineidence of adverse reactions between the Iwo groups(P>0.05).Conc lus ion Compared with the single use of ACEI and ARB.fasudil combined with ACEI and ARB is more efete in alviaing symploms and improv ing cardiac function in elderly patients with CHF,without increasing adverse reactions.I1 is worthy of popularization and application without serious adverse reactions,safe and feasible.
