通心络胶囊对冠心病患者血管内皮功能影响的Meta分析

摘　　要：目的系统评价通心络胶囊对冠心病患者血管内皮功能的影响。方法通过检索中国知网(CNKI)、万方数据库(Wan Fang Data)、维普数据库(VIP)、中国生物医学文献数据库(CBM)、PubMed、Cochrane Library、EMbase,检索年限均为建库至2021年6月30日。2名研究人员独立阅读和筛查,按照纳入标准筛选提取文献,对纳入的文献采用Cochrane推荐的偏倚风险评估方法进行偏倚评价,采用RevMan5.3软件进行Meta分析。结果最终纳入23项随机对照试验(RCT),总病例数2103例。Meta分析结果显示,与常规西药治疗(对照组)比较,通心络胶囊联合常规西药治疗(观察组)患者血管性假血友病因子(vWF)、血清内皮素(ET)、血栓素B2(TXB2)、可溶性血管细胞黏附分子-1(sVCAM-1)水平均降低[vWF:MD=-26.35,95%CI(-32.20,-20.50),P<0.01;ET:MD=-16.85,95%CI(-20.40,-13.31),P<0.01;TXB2:MD=-9.08,95%CI(-11.28,-6.88),P<0.01;sVCAM-1:MD=-36.33,95%CI(-44.46,-28.21),P<0.01];一氧化氮(NO)、血管内皮生长因子(VEGF)、血管内皮依赖性舒张功能(FMD)均升高[NO:MD=10.36,95%CI(8.86,11.86),P<0.01;VEGF:MD=61.69,95%CI(16.22,107.17),P<0.01;FMD:MD=1.51,95%CI(0.86,2.15),P<0.01];而2组血管内皮非依赖性舒张功能(NMD)比较差异无统计学意义[MD=1.70,95%CI(-0.85,4.25),P=0.19]。结论通心络胶囊联合常规西药治疗可显著改善冠心病血管内皮功能,且疗效确切。Objective To systematically review the effect of Tongxinluo capsule on vascular endothelial function in patients with coronary heart disease.Methods By searching China National Knowledge Infrastructure(CNKI),Wan Fang Data,VIP database(VIP),China Biomedical Literature Database(CBM),PubMed,Cochrane Library,EMbase,the search period was established.Library until June 30,2021.Two researchers independently read and screened,and selected and extracted literature according to the inclusion criteria.The included literatures were assessed for bias using the bias risk assessment method recommended by Cochrane,and RevMan5.3 software was used for meta-analysis.Results A total of 23 randomized controlled trials(RCTs)were included,with a total of 2103 cases.Meta-analysis results showed that compared with conventional western medicine treatment(control group),Tongxinluo capsule combined with conventional western medicine treatment(observation group)patients with von Willebrand factor(vWF),serum endothelin(ET),thromboxane B2(TXB2)and soluble vascular cell adhesion molecule 1(sVCAM1)levels were decreased[vWF:MD=-26.35,95%CI(-32.20,-20.50),P<0.01;ET:MD=-16.85,95%CI(-20.40,-13.31),P<0.01;TXB2:MD=-9.08,95%CI(-11.28,-6.88),P<0.01;sVCAM1:MD=-36.33,95%CI(-44.46,-28.21),P<0.01];nitric oxide(NO),vascular endothelial growth factor(VEGF),and vascular endothelial-dependent relaxation function(FMD)were all elevated[NO:MD=10.36,95%CI(8.86,11.86),P<0.01;VEGF:MD=61.69,95%CI(16.22,107.17),P<0.01;FMD:MD=1.51,95%CI(0.86,2.15),P<0.01];while the two groups of vascular endothelium-independent relaxation There was no significant difference in function(NMD)[MD=1.70,95%CI(-0.85,4.25),P=0.19].Conclusion Tongxinluo capsule combined with conventional western medicine can significantly improve the endothelial function of coronary heart disease,and the curative effect is definite.
