前列地尔联合负压封闭引流治疗对糖尿病足患者氧化应激和炎症反应的影响

摘　　要：目的探讨前列地尔联合负压封闭引流治疗对糖尿病足患者氧化应激和炎症反应的影响。方法选取2019年7月—2020年6月收治的糖尿病足96例为研究对象,根据治疗方法不同分为观察组和对照组,每组48例。在常规治疗基础上,对照组给予前列地尔治疗,观察组给予前列地尔联合负压封闭引流治疗。比较两组治疗前后血糖、氧化应激和炎症相关指标,同时观察两组的治疗效果及不良反应发生情况。结果治疗后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白、丙二醛(MDA)、晚期蛋白氧化产物(AOPPs)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和C反应蛋白(CRP)水平较治疗前降低,超氧化物歧化酶(SOD)水平较治疗前升高,且观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、MDA、AOPPs、IL-6、TNF-α和CRP水平均低于对照组,SOD水平高于对照组(P<0.05,P<0.01)。观察组总有效率高于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论在常规治疗基础上,采用前列地尔联合负压封闭引流可以显著减轻糖尿病足患者氧化应激和炎症反应,提高治疗总有效率,而且安全可靠。Objective To investigate effects of Alprostadil combined with vacuum sealing drainage on oxidative stress and inflammatory reaction in patients with diabetic foot.Methods A total of 96 patients with diabetic foot admitted between July 2019 and June 2020 were selected and divided into observation group(n=48)and control group(n=48)according to different therapeutic methods.On the basis of conventional treatment,control group was treated with Alprostadil,while observation group was treated with Alprostadil combined with vacuum sealing drainage.Blood glucose,and related indexes of oxidative stress and inflammation before and after treatment were compared,and therapeutic effects and adverse reactions were recorded in two groups.Results After treatment,levels of fasting blood glucose,2h postprandial blood glucose(2h PBG),glycosylated hemoglobin,malondialdehyde(MDA),advanced protein oxidation products(AOPPs),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)levels were significantly lower,while levels of superoxide dismutase(SOD)were significantly higher than those before treatment in two groups,compared with those in control group,levels of fasting blood glucose,2h PBG,glycosylated hemoglobin,MDA,AOPPs,IL-6,TNF-αand CRP were significantly lower,while SOD level was significantly higher in observation group(P<0.05,P<0.01).The total effective rate in observation group was significantly higher than that in control group(P<0.05).There was no significant difference in incidence rate of adverse reactions between two groups(P>0.05).Conclusion On the basis of conventional treatment,Alprostadil combined with vacuum sealing drainage may significantly reduce oxidative stress and inflammatory reaction and improve the total effective rate with safe and reliable in treatment of patients with diabetic foot.
