穴位埋线联合通络益肾汤治疗坐骨神经痛疗效及对患者痛阈值与IL-6表达水平影响研究

摘　　要：目的:探讨穴位埋线联合通络益肾汤治疗坐骨神经痛的疗效及对患者痛阈值与血清白细胞介素-6(IL-6)水平的影响。方法:选择医院收治的坐骨神经痛患者89例,随机分为对照组(44例)和研究组(45例)。两组均给予阿司匹林缓解症状,维生素B12营养神经等基础治疗,对照组给予内服和外敷通络益肾汤治疗,研究组在此基础上给予穴位埋线治疗。比较两组患者治疗前后中医证候积分,比较两组患者治疗前及治疗1、2、3个月后痛阈值、血清IL-6变化及临床疗效。结果:治疗1、2、3个月后,两组患者中医证候积分下降,研究组下降较明显(P<0.05),治疗1、2、3个月后,两组患者痛阈值水平均呈现显著升高趋势,血清IL-6水平呈现显著下降趋势(P<0.05);研究组治疗1、2、3个月后痛阈值水平均高于对照组,血清IL-6水平均低于对照组(P<0.05);治疗后,两组患者疲乏评分增高,抑郁指数下降,且研究组改善较明显(P<0.05)。研究组总有效率(97.80%)明显高于对照组(84.09%)(P<0.05)。结论:应用穴位埋线联合通络益肾汤治疗坐骨神经痛疗效满意,可显著提高患者疼痛阈值,降低血清促痛因子表达水平,改善患者疼痛症状。Objective:To investigate the effect of acupoint embedding combined with Tongluo Yishen Decoction on sciatica and the effect on pain threshold and serum IL-6 level.Methods:89 patients with sciatica were selected.The method of random number was used to divide it into control group(44 cases)and study group(45 cases).Both groups were given basic treatment such as aspirin to alleviate symptoms and vitamin B12 to nourish nerves.The control group was given internal and external application of Tongluo Yishen Decoction,and the study group was given acupoint catgut embedding treatment.The syndrome score of TCM before and after treatment was compared between the two groups patients.The pain threshold,serum IL-6 and clinical effect were compared before and after 1,2,3 months.Results:After 1,2,3 months,the TCM syndrome score of the two groups patients decreased,and the study group was more obvious(P<0.05).After 1,2,3 months,the pain threshold level of the two groups patients increased significantly,and the serum IL-6 level showed a significant decline(P<0.05);The pain threshold level in the study group was higher than that of the control group after 1,2 and 3 months,and the serum IL-6 was lower than that of the control group(P<0.05);After treatment,the fatigue score of the two groups patients increased,the depression index decreased,and the study group was more obvious(P<0.05).The total effective rate of the study group(97.80%)was significantly higher than that of the control group(84.09%)(P<0.05).Conclusion:The effect of acupoint catgut embedding combined with Tongluo Yishen Decoction on sciatica is satisfactory,which can significantly improve the pain threshold,reduce the expression level of serum pain promoting factor and improve the pain symptoms of patients.
