独活寄生汤联合自体富血小板血浆治疗风寒湿痹型膝骨性关节炎临床观察

摘　　要：目的:探讨独活寄生汤联合自体富血小板血浆治疗风寒湿痹型膝骨性关节炎(KOA)患者的临床效果。方法:选取KOA患者120例,采用随机数字表法分为对照组和治疗组两组,每组各60例。对照组应用PRP治疗,治疗组在对照组治疗的基础上加用独活寄生汤,对比两组炎症因子水平、关节功能、疼痛程度及不良反应。结果:治疗后,治疗组总有效率93.3%,明显高于对照组(80.0%)P<0.05;治疗组患者的白介素-6(IL-6)为(1.92±0.25)ng/mL、肿瘤坏死因子α(TNF-α)为(4.08±0.82)pg/mL、白介素-1β(IL-1β)为(0.23±0.05)μg/mL、安大略西部和麦克马斯特大学骨关节炎指数(WOMAC)评分为(92.68±17.87)分、视觉模拟评分(VAS)为(3.32±0.54)分,分别低于对照组的相应指标:(2.73±0.37)ng/mL、(6.62±0.97)pg/mL、(0.46±0.09)μg/mL、(114.46±16.32)分、(3.98±0.60)分,差异均有统计学意义(P<0.05)。两组治疗期间未见明显不良反应。结论:独活寄生汤联合自体富血小板血浆治疗风寒湿痹型膝骨性关节炎疗效显著,可减轻患者炎症反应、疼痛程度,改善关节功能,值得临床推广。Objective:To investigate the clinical effect of Duhuo Jisheng decoction combined with autologous plateletrich plasma in the treatment of knee osteoarthritis(KOA)of wind cold dampness type.Methods:120 patients with KOA who were treated in our hospital from August 2018 to August 2020 were selected and divided into two groups by random number table method,with 60 patients in each group.The control group was treated with PRP,and the treatment group was treated with Duhuo Jisheng decoction.The levels of inflammatory cytokines,joint function,pain degree and adverse reactions were compared between the two groups.Results:After treatment,the total effective rate of the treatment group was 93.3%,which was significantly higher than that of the control group(80.0%,P<0.05).Interleukin-6(IL-6)(1.92±0.25)ng/mL,tumor necrosis factorα(TNF-α)(4.08±0.82)pg/mL,interleukin-1β(IL-1β)(0.23±0.05)μg/mL,Western Ontario and McMaster University Osteoarthritis Index(WOMAC)in the treatment group)score(92.68±17.87),visual analogue scale(VAS)score(3.32±0.54),Compared with the control group(2.73±0.37)ng/mL,(6.62±0.97)pg/mL,(0.46±0.09)μg/mL,(114.46±16.32)points,(3.98±0.60)points,there was statistical difference(P<0.05).There were no obvious adverse reactions in the two groups during treatment.Conclusion:Duhuo Jisheng decoction combined with autologous platelet-rich plasma has a significant effect on the treatment of patients with knee osteoarthritis of wind cold dampness type,which is beneficial to reduce inflammatory reaction,pain degree and improve joint function,and is worthy of clinical promotion.
