摘  要:目的:探讨扶肺固肾饮对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)大鼠气道炎症及糖皮质激素受体的影响。方法:50只雄性Wistar大鼠,随机分为5组:空白对照组、COPD模型组及扶肺固肾饮高、中、低剂量组,每组10只,除空白对照组,其余4组以烟熏联合脂多糖、冷空气刺激制造COPD大鼠模型。造模成功后,空白对照组、COPD模型组予蒸馏水3 mL/只灌胃,扶肺固肾饮高、中、低剂量组予1.02、0.51、0.26 g中药配方颗粒/100 g/天,每日2次灌胃,连续给药28 d。采集样本,观察苏木素-伊红染色肺组织的病理变化,酶联免疫吸附法检测各组大鼠血清和右肺泡灌洗液(BALF)中肿瘤坏死因子α(TNF-α)、转化生长因子β1(TGF-β1)、白细胞介素-17A(IL-17A)的含量以及左肺组织中基质金属蛋白酶(MMP-9)、金属蛋白酶-1组织抑制因子(TIMP-1)表达的水平,实时荧光定量聚合酶链式反应测定大鼠左肺组织糖皮质激素受体(GR)mRNA含量,免疫组化测定左肺组织GR的表达水平。结果:扶肺固肾饮高、中、低剂量组COPD大鼠血清及BALF中TNF-α、TGF-β1、IL-17A的含量均较COPD模型组明显减少(P<0.05);大鼠肺组织中TIMP-1、MMP-9的表达均较COPD模型组减少(P<0.05);肺组织中GR mRNA含量、GR的表达较COPD模型组升高(P<0.05),尤以中剂量组疗效最为显著。结论:扶肺固肾饮可抑制COPD大鼠肺组织中炎症因子的释放,改善气道炎症及重塑,提高激素的敏感性。Objective:To investigate the effect of Fufei Gushen Decoction on airway inflammation and glucocorticoid re-ceptor in rats with chronic obstructive pulmonary disease(COPD).Methods:Fifty Wistar male rats were randomly divided into 5 groups:the blank control group,the COPD model group,the high,the medium and lthe ow dose groups of Fufei Gushen Yin De-coction,with 10 rats in each group.Except the blank control group,the remaining 4 groups were treated by smoked combined with lipopolysaccharide(LPS)and cold air stimulation to establish CODP rat model.After successful modeling,the blank control group and the COPD model group were gavaged with distilled water 3 mL,while the high,medium and low dose groups of Fufei Gushen Yin Decoction were given 1.02,0.51,0.26 g granules of of Fufei Gushen Yin Decoction/100 g/day,respectively,2 times a day for 28 consecutive days.Samples were collected,hematoxylin-eosin staining was used to observe the pathological changes of lung tissue.Enzyme-linked immunosorbent assay was used to detect the tumor necrosis factor alpha(TNF-α),trans-forming growth factorβ1(TGF-β1),and interleukin-17(IL-17A)in the serum and right alveolar lavage fluid(BALF),and ma-trix metalloproteinase(MMP-9)and tissue inhibitor of metalloproteinase-1(TIMP-1)in the left lung tissue.Real-time fluores-cence quantitative polymerase chain reaction and immunohistochemistry were used to detect mRNA and protein level of glucocorti-coid receptor(GR)in left lung tissue.Results:The content of TNF-α,TGF-β1,and IL-17A in the serum and BALF of COPD rats in the Fufei Gushen Yin high,medium and low dose groups were significantly reduced compared with the COPD model group(P<0.05).The expressions of TIMP-1 and MMP-9 in tissues were lower than those in the COPD model group(P<0.05).The mRNA and protein expressions of GR in lung tissues were higher than those in the COPD model group(P<0.05)and the middle-and low-dose groups,and the middle-dose group had the most significant effect.Conclusion:Fufei Gushen Decoction can inhibit the rel
