摘　　要：目的评估后部眼球筋膜下注射曲安奈德联合玻璃体内注射雷珠单抗治疗弥漫性糖尿病性黄斑水肿(DME)的效果及安全性。方法回顾性分析2019年4月至2021年4月郑州市第七人民医院弥漫性DME 60例(60眼)的临床资料。所有患者根据治疗方式分为两组,各30例(30眼)。单纯行雷珠单抗玻璃体内注射者为对照组,曲安奈德注射液眼后部球筋膜下注射联合雷珠单抗注射液玻璃体内注射者为试验组。注射后随访6个月进行两组效果比较。结果对照组与试验组注射后1周、1、3及6个月视力明显较注射前提高(均P<0.05),试验组均优于对照组(均P<0.05),两组黄斑中心区厚度均低于治疗前(均P<0.05);试验组均低于对照组(均P<0.05)。结论眼后部球筋膜下注射曲安奈德注射液联合玻璃体内注射雷珠单抗注射液治疗弥漫性DME可更有效的减轻视网膜水肿并改善视力。Objective To evaluate the efficacy and safety of posterior sub-tenon’s injection of triamcinolone acetonide combined with intravitreal injection of ranibizumab for the treatment of diffuse diabetic macular edema(DME).Methods The clinical data of 60 eyes of 60 cases of DME in Zhengzhou Seventh People’s Hospital from Apr.2019 to Apr.2021 were analyzed retrospectively.All patients were divided into the two groups based on the treatment method.There were 30 eyes of(30 patients)in each group.The control group received intravitreal injection of ranibizumab alone,and the trial group received posterior sub-tenon’s injection of triamcinolone acetonide combined with intravitreal injection of ranibizumab.At 6 months of follow-up,the efficacy between the two groups were compared.Results The visual acuity of the two groups at 1 week,1 month,3 and 6 months after injection were significantly improved compared with that before injection(all P<0.05).The visual acuity of the trial group was better than that of the control group(all P<0.05).The central macular thickness of the two groups was lower than that before treatment(all P<0.05).Conclusion Posterior sub-tenon’s injection of triamcinolone acetonide combined with intravitreal injection of ranibizumab for the treatment of diffuse DME is more effective in reducing retinal edema and improving visual acuity.
