钩活术治疗颈椎病合并颈肩痛临床观察

摘　　要：目的观察钩活术治疗颈椎病合并颈肩痛的临床疗效。方法将180例颈椎病合并颈肩痛患者按照随机数字表法分为2组,治疗组90例(实际完成85例)予钩活术治疗,对照组90例(实际完成84例)予常规针刺治疗。2组均治疗7 d后统计疗效,比较2组治疗前后疼痛视觉模拟评分法(VAS)和压痛阈水平变化情况,比较2组治愈患者治疗1年后复发情况。结果治疗组总有效率98.82%(84/85),对照组90.48%(76/84),治疗组疗效优于对照组(P<0.05)。与本组治疗前比较,2组治疗后疼痛VAS均降低(P<0.05),压痛阈值均升高(P<0.05),且治疗组治疗后改善均优于对照组(P<0.05)。治疗组治愈患治疗1年后复发率3.85%(2/52),对照组15.63%(5/32),治疗组复发率低于对照组(P<0.05)。结论钩活术治疗颈椎病合并颈肩痛临床疗效确切,可明显改善患者疼痛症状,提高疼痛敏感度,且远期疗效稳定持久,复发率低,安全可靠。Objective To observe the clinical effect of hook operation for cervical spondylosis(CS)with neck and shoulder pain.Methods Totally 180 CS patients with neck and shoulder pain were randomly divided into equal group,90 cases(85 cases completed)in treatment group were treated with hook operation,and 90 cases(84 cases completed)in control group with routine acupuncture treatment.A 7-day treatment were performed to compare curative effect,visual analogue scale(VAS),pressure pain threshold(PPT);recurrence rate on year 1 was counted.Results The total effective rates in treatment group and control group were 98.82%(84/85)and 90.48%(76/84),respectively,the curative effect being better in treatment group than control group(P<0.05).After treatment,VAS in groups was decreased(P<0.05),while PPT was increased(P<0.05),improvement of VAS and PPT in treatment group were significantly better(P<0.05).The recurrence rate on year 1 was lower in treatment group than in control group[3.85%(2/52)vs 15.63%(5/32),(P<0.05),respectively].Conclusion For CS patients with neck and shoulder pain,hook operation has a definite clinical effect,which is significantly capable of improving pain symptoms and pain sensitivity,with stable and lasting long-term efficacy,low recurrence rate,and safe and reliable.
