摘  要:目的:探讨3.0T MRI3D-VIBE序列联合OsiriX软件半自动分割对测量膝骨关节炎软骨体积的效率和可重复性。方法:随机选取2018年1月-2020年12月期间在本院影像科接受3.0T MRI3D-VIBE序列检查并符合条件的患者80例,分为轻度KOA组(39例)和重度KOA组(41例),同时纳入健康志愿者40例为对照组。对MR图像质量进行一致性检验;使用OsiriX软件分割比较三组受试者的各膝关节软骨体积;比较OsiriX软件半自动分割和常规手工分割在测量KOA组和对照组的软骨体积所需时间和可重复性误差。结果:3D-VIBE序列膝关节软骨成像的图像质量评分结果一致性较好(Kappa值=0.83,P<0.01);轻度KOA组各软骨体积值均比对照组低,但都高于重度KOA组,所有比较均有显著统计学差异(P<0.01);KOA组和对照组使用OsiriX软件半自动分割膝关节软骨所用时间都比常规手工测量少,差异均有统计学意义(P<0.01);KOA组和对照组使用OsiriX软件半自动分割和常规手工测量比较,观察者间可重复测量误差和观察者内可重复测量误差均低于常规手工测量(P<0.05)。结论:3.0T MRI3D-VIBE序列成像对于显示膝关节软骨图像质量良好,其联合OsiriX软件半自动测量对于分割膝关节各软骨比常规手工测量更节约时间,可重复性更好。Objective:To explore the efficiency and repeatability of 3.0T MRI 3D-VIBE sequence combined with OsiriX software semi-automatic segmentation in measuring cartilage volume of knee os-teoarthritis.Methods:80 patients who received 3.0T MRI 3D-VIBE sequence examination in the imag-ing department of our hospital from January 2018 to December 2020 were randomly divided into mild KOA group(n=39)and severe KOA group(n=41),and 40 healthy volunteers were included as con-trol group.The consistency of MR image quality was tested;the knee cartilage volumes of the three groups were compared by OsiriX software segmentation;the time and repeatability errors of OsiriX soft-ware semi-automatic segmentation and conventional manual segmentation in measuring cartilage vol-ume between KOA group and control group were compared.Results:the image quality scores of 3.0TM-RI3D-VIBE sequence knee cartilage imaging were consistent(Kappa=0.83,P<0.0l),and the cartilage volume values of mild KOA group were lower than those of control group,but higher than those of se-vere KOA group(P<0.0l).The time of semi-automatic segmentation of knee cartilage by OsiriX software in KOA group and control group was less than that by routine manual measurement,and the difference was statistically significant(P<0.01).Compared with routine manual measurement and semi-automatic segmentation with OsiriX software,the inter-observer and intra-observer repeatable measurement errors in KOA group and control group were lower than those in conventional manual measurement(P<0.05).Conclusion:3.0TMRI3D-VIBE sequence imaging has good image quality for displaying knee cartilage,and its combination with OsiriX semi-automatic software measurement is more time-saving and repeat-able than conventional manual measurement in segmenting knee cartilage,so it is worth popularizing.
