彩色多普勒超声对凶险性前置胎盘合并胎盘植入的诊断价值

摘　　要：目的:通过对凶险性前置胎盘合并植入的彩色多普勒超声图像的分析,探究其诊断意义。方法:收集2019年1月-2021年4月在我院产前彩色多普勒超声怀疑凶险性前置胎盘合并植入,后经住院剖宫产证实为凶险性前置胎盘的病例共41例,其中合并植入32例,回顾性分析其声像图特点。结果:在41例病例中,术中诊断未合并植入9例(21.95%),合并植入32例(78.05%),植入病例中胎盘粘连5例,胎盘浅植入7例,胎盘穿透性植入20例,其中穿透植入并累及膀胱2例。在采用不同途径超声检查对植入诊断符合率上,单纯经腹部超声检查符合率84.38%,而经腹部联合经阴道超声检查达93.57%的符合率。两者相比,经腹部联合经阴道明显高于前者,差异有统计学意义(P<0.05)。结论:彩色多普勒超声经腹部联合经阴道对凶险性前置胎盘合并胎盘植入可作出正确诊断,为临床诊治提供依据,是值得推荐的首选检查手段。Objective:To explore the diagnostic significance of pernicious placenta previa with pla-centa accreta by color Doppler ultrasound image analysis.Methods:From January 2019 to April 2021,41 cases of pernicious placenta previa complicated with placenta accreta suspected by prenatal color Doppler ultrasound in our hospital were collected,including 32 cases complicated with placenta accre-ta,The sonographic features were analyzed retrospectively.Results:Among the 41 cases,9 cases(21.95%)were not complicated with placenta accreta and 32 cases(78.05%)were complicated with pla-centa accreta.Among the 32 cases of placenta accreta,there were 5 cases of Placenta Adhesion,7 cas-es of superficial placenta accreta and 20 cases of penetrating placenta accreta.Among them,2 cases of penetrating placenta accreta involved the bladder。The coincidence rate of transabdominal ultrasound alone was 84.38%,while that of transabdominal combined with transvaginal ultrasound was 93.75%.The coincidence rate of transabdominal combined with transvaginal ultrasound was significantly higher than that of transabdominal combined with transvaginal ultrasound(P<0.05).Conclusion:Color doppler ultrasound transabdominal combined with transvaginal in the diagnosis of pernicious placenta previa with placenta accreta can make a correct diagnosis, provide the basis for clinical diagnosis and treatment, is the preferred examination method worthy of recommendation.
