中医推拿对LBP患者腰肌组织温度及血清5⁃HT和β⁃EP的影响

摘　　要：目的:分析下腰痛(LBP)患者经中医推拿治疗后其腰部疼痛(VAS)、腰椎功能(JOA)、肌组织温度及血清中5-羟色胺(5-HT)和β-内啡肽水平(β-EP)的变化。方法:将64例在本院推拿科治疗的LBP患者,随机分为观察组32例、对照组32例。对照组采用传统毫针刺法治疗,观察组采用推拿手法治疗。比较两组患者治疗前和治疗后其下腰部疼痛、腰椎功能、肌组织温度及5-HT和β-EP水平的差异。结果:治疗后,观察组下腰部疼痛VAS评分、腰椎功能JOA评分均明显低于对照组(P<0.01),观察组与对照组体温上升明显(P<0.01),且观察组肌组织温度高于对照组,二者差异具有统计学意义(P<0.05);治疗前后5-HT和β-EP水平的差异具有统计学意义(P<0.01),且观察组5-HT、β-EP水平明显高于对照组,差异具有统计学意义(P<0.05)。结论:推拿治疗LBP临床疗效显著,采用医用红外热成像技术和血清疼痛物检测技术,有助于客观分析推拿治疗疾病的确切疗效和对相关指标的影响。Objective:To analyze the level changes of the degree of back pain by visual analogue scale(VAS),lumbar function(Japanese orthopaedic association scores,JOA),muscle temperature,serum serotonin 5-hydroxytryptamine(5-HT)andβ-endorphin(β-EP)in patients with low back pain(LBP).Methods:A total of 64 LBP patients treated in the Massage De-partment of The Ffirst Affiliated Hospital of Hainan Medical University were randomly divided into observation group(32 cases)and control group(32 cases).The control group was treated with traditional filiform acupuncture needling;the observation group was treated with Chinese tuina.VAS,JOA,muscle temperature,5-HT,andβ-EP were tested before and after treatment.Re⁃sults:After treatment,the VAS and JOA score in the observation group were significantly lower than those in the control group(P<0.01),the muscle temperature in the observation group and the control group increased significantly(P<0.01),and muscle temperature in the observation group was higher than that in the control group(P<0.05);5-HT andβ-EP were significantly dif-ferent before and after treatment(P<0.01),and the level in the observation group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Chinese tuina has a significant clinical efficacy in the treatment of LBP.The medical infrared thermal imaging technology and serum pain detection technology are conducive to objec-tively analyzeing the exact efficacy of tuina in the treatment of LBP and its impact on relevant indexes.
