摘　　要：目的探讨第二趾短伸趾肌腱转位加强拇内收肌止点重建治疗拇外翻的疗效。方法将14例拇外翻患者采用第二趾短伸趾肌腱转位加强拇内收肌止点重建治疗。记录拇外翻角、跖骨间角。采用AOFAS踝-后足评分评价临床疗效。结果患者均获得随访,时间6~18个月。术后1周、6个月,拇外翻角分别为5°~10°(8.2°±3.1°)、5°~15°(10.2°±5.3°);跖骨间角分别为5°~10°(7.1°±3.2°)、5°~15°(8.2°±6.3°)。末次随访时AOFAS踝-后足评分86~92(89.0±3.4)分;拇外翻畸形均得到有效纠正,行走疼痛完全缓解。结论利用第二趾短伸趾肌腱转位加强拇内收肌在第1跖骨颈止点重建治疗拇外翻,通过动力平衡能持久稳定跖骨间角,可矫正拇外翻畸形,缓解患者行走疼痛,有效预防畸形复发。Objective To investigate the effect of translocation of extensor tendon of the second toe for strengthening the insertion reconstruction of adductor hallucis in the treatment of hallux valgus.Methods Fourteen patients with hallux valgus were adopted with translocation of extensor tendon of the second toe for strengthening the insertion reconstruction of adductor hallucis.The hallux valgus angle and intermetatarsal angle were recorded.Clinical efficacy was evaluated by AOFAS ankle-hindfoot score.Results All patients were followed up for 6~18 months.At 1 week and 6 months after surgery,the valgus angles were 5°~10°(8.2°±3.1°)and 5°~15°(10.2°±5.3°),respectively;the intermetatarsal angles were 5°~10°(7.1°±3.2°)and 5°~15°(8.2°±6.3°),respectively.At the last follow-up,the AOFAS ankle-hindfoot score ranged from 86~92(89.0±3.4)points.All the hallux valgus deformities were corrected effectively,and walking pain was completely relieved.Conclusions The translocation of extensor tendon of the second toe for strengthening the insertion reconstruction of adductor hallucis at the neck of the first metatarsal bone are used to treat hallux valgus,which can maitain persistently the intermetatarsal angle through dynamic balance,and correct the hallux valgus deformity,relieve the walking pain of patients,and effectively prevent the recurrence of the deformity.
