经尿道刀口变向钬激光膀胱黏膜剥脱术治疗女性腺性膀胱炎临床观察

摘　　要：目的观察经尿道刀口变向钬激光膀胱黏膜剥脱术(THoLBMS)治疗女性腺性膀胱炎的临床效果。方法腺性膀胱炎女性患者67例,其中35例接受THoLBMS治疗(THoLBMS组),32例接受经尿道等离子汽化术(TUV)治疗(TUV组)。观察并比较两组患者手术时间、术后膀胱冲洗时间、留置导尿管时间及住院时间,手术过程中有无膀胱穿孔及闭孔反射,术前术后血红蛋白(HGB)和Na^(+)变化值,术后有无出现膀胱刺激征状、术后6个月时膀胱炎症量表(ICSI)评分。术后6个月进行疗效评价。结果THoLBMS组患者治疗有效34例、无效1例,TUV组患者治疗有效26例、无效6例,两组相比,P<0.05。THoLBMS组患者术前术后HGB变化值、术后出现膀胱刺激征者、术后6个月时ICSI评分均低于TUV组(P均<0.05)。结论与TUV相比,THoLBMS治疗女性腺性膀胱炎疗效好、出血少、并发症少。Objective To observe the clinical effect of transurethral holmium laser bladder mucosal exfoliation(THoLBMS)in the treatment of female cystitis glandularis.Methods Sixty-seven female patients with cystitis glandularis were treated with transurethral holmium laser bladder mucosal exfoliation(THoLBMS)(n=35)and transurethral plasma vaporization(TUV)(n=32).The operation time,postoperative bladder irrigation time,indwelling catheter time and hospitalization time,bladder perforation and obturator reflex during operation,changes of hemoglobin(HGB)and Na^(+)before and after operation,bladder irritation symptoms after operation and interstitial cystitis symptom index(ICSI)score at 6 months after operation were observed and compared between the two groups.The curative effect was evaluated at 6 months after operation.Results In the THoLBMS group,34 cases were effective and 1 case was ineffective,while in the TUV group,26 cases were effective and 6 cases were ineffective(both P<0.05).The changes of HGB before and after operation,the postoperative bladder irritation syndrome and the ICSI score at 6 months after operation in the THoLBMS group were all lower than those in the TUV group(all P<0.05).Conclusion Compared with TUV,THoLBMS has better efficacy,less bleeding and fewer complications in the treatment of female cystitis glandularis.
