摘 要:生物标志物在慢性阻塞性肺疾病(COPD)诊断、判断疾病严重程度、评估预后、预测药物临床疗效和了解疾病病理生理中有重要作用。目前涉及COPD患者多种遗传、生化等方面的生物标志物的研究较多,尽管许多研究结果显示差异有统计学意义,但研究中存在的诸多问题限制了生物标志物在临床实践中的应用。本文分析了COPD生物标志物在研究设计、诊断标准方面的局限性。以肺气肿表型生物标志物研究为例,阐述了相关研究的复杂性。并对寻找具有临床指导意义的生物标志物的相关研究提出了建议。Biomarkers play an important role in the diagnosis of chronic obstructive pulmonary disease(COPD),in judging the severity of the disease,evaluating the prognosis,predicting the clinical efficacy of drugs,and understanding the pathophysiology of the disease.At present,there are many studies involving multiple genetic and biochemical biomarkers in patients with COPD.Although many research results show statistically significant differences,the problems in the studies limit the application of biomarkers in clinical practice.The study design and diagnostic criteria limitations of COPD biomarkers were analyzed.Taking the study of phenotypic biomarkers of emphysema as an example,the complexity of related research was expounded.In addition,suggestions are made as to finding biomarkers with clinical guiding significance.
