Allium覆膜金属支架治疗恶性肿瘤引起的输尿管梗阻治疗的初步应用体会

摘　　要：目的:初步评估Allium覆膜金属支架用于恶性肿瘤性输尿管梗阻的安全性和有效性。方法:回顾性分析2019年12月—2020年10月在重庆医科大学附属永川医院9例因恶性肿瘤性输尿管梗阻行内镜下Allium覆膜金属支架置入治疗的患者的临床资料。术后第1、3、6、12个月复查血肌酐、超声下肾盂宽度,并通腹部X线检查观察支架位置和形态有无变化。观察统计患者的支架长期通畅率、并发症发生率、肾功能变化和肾积水情况。结果:所有9例患者均成功置入Allium覆膜支架。术后随访3~12个月。所有患者在随访期间支架均保持位置正常和输尿管通畅,2例肌酐异常患者肌酐较术前好转,8例患肾盂积水较术前明显缓解,仅2例患者出现ClavienⅠ并发症。结论:Allium覆膜金属输尿管支架治疗恶性肿瘤性输尿管梗阻是安全有效的,可以保持相对长期的通畅率,有助于保护肾功能,提高患者生活质量。Objective: To summarize the initial clinical experience and follow-up results of the treatment for malignant ureteral obstruction with Allium coated metal ureteral stent. Methods: From December 2019 to October 2020, 9 patients with malignant ureteral obstruction underwent Allium ureteral stent insertion in Yongchuan Hospital, Chongqing Medical University. The serum creatinine and BUN, renal pelvis width under ultrasound, urography and abdominal plain film(KUB) were reviewed at the end of 1, 3, 6 and 12 months and annually postoperatively to observe the stent position and morphology. The long-term stent patency rate, complication rate, renal function and hydronephrosis were followed up and analyzed. Results: All the 9 cases in this group were successfully implanted. The patients were followed up for 3 to 12 months. All patients maintained normal position and ureteral patency during the follow-up period. Creatinine was improved in 2 patients with abnormal creatinine, and hydronephrosis was relieved in 8 patients. Postoperative Clavien Ⅰ complications occurred in 2 patients. Conclusion: Allium coated metal ureteral stent could be used for the treatment for malignant ureteral obstruction, which could maintain relatively long-term patency rate and protect renal function. The indwelling time was longer and it could improve quality of life of patients.
