针刺辅助治疗重症急性胰腺炎的研究现状及展望

摘　　要：重症急性胰腺炎(SAP)是临床常见的急腹症之一,以其起病急、病情复杂多变、致死率高的特点,一直以来受到医学界广泛关注。单纯西医治疗具有一定的局限性,目前中西医结合在治疗SAP方面取得良好效果,并已成为治疗SAP的重要手段,中医在其中发挥着重要作用。针刺疗法是中医治疗方法中的重要组成部分,针刺辅助治疗SAP在镇痛、促进肠蠕动、抑制炎症反应、保护胰外器官等方面治疗效果明显,且针刺具有操作简便、廉价、无任何不良反应等特点,因此近年来得到越来越广泛的应用,具有良好的临床推广价值。大量的临床和基础研究都证实,针刺可以通过不同的作用机制对SAP发挥辅助治疗作用,且针刺辅助治疗在针刺方法、针刺时间、针刺穴位等方面尚缺乏统一标准。鉴于此,本文总结了近年来针刺辅助治疗SAP相关机制研究和临床研究,为进一步拓展针刺的临床治疗范围提供参考。Severe acute pancreatitis(SAP)is one of acute abdominal diseases commonly seen in the clinic,and its characteristics are rapid onset,disease situation complex and changeable with high mortality,thus it has been widely concerned by the medical field.Simple Western medicine for SAP therapy has certain limitations.At present,the combination of Chinese and Western medicine for treatment of SAP has become an important method and obtained excellent therapeutic results,and in the combination,Chinese medicine plays a valuable role.Acupuncture is an important constituent part of traditional Chinese medicine treatment.It is extensively used because of its method of simple manipulation,cost low and generally no toxic and side effects;acupuncture used as an auxiliary treatment of SAP possesses the advantages in analgesia,promoting intestinal peristalsis,inhibiting inflammatory response and also protecting organs outside the pancreas,therefore,at present,acupuncture has been used more and more widely in the treatment of SAP as an adjuvant with good clinical promotional value.A large number of clinical and basic studies have demonstrated that acupuncture can play an auxiliary role via different mechanisms in treatment of SAP,however,the mechanism of acupuncture for treatment of SAP has not yet been elucidated,and there is still a lack of unified standards of acupuncture in methods,time,acupoints,etc.Thus in this paper,the experimental and clinical studies related to the mechanism and clinical application of acupuncture as an auxiliary treatment for SAP in recent years were summarized in order to provide references for further clarifying the mechanism of acupuncture and expanding its clinical treatment scope.
