射频热凝术联合胶原酶化学溶解术治疗青少年腰椎间盘突出症的效果

摘　　要：目的比较射频热凝术联合低剂量胶原酶化学溶解术与单侧开窗髓核摘除术治疗青少年腰椎间盘突出症的效果。方法选取2019年6月-2020年6月我院收治的青少年腰椎间盘突出症患者65例,根据治疗方式不同分为A组33例,B组32例。A组予以射频热凝术联合胶原酶化学溶解术治疗,B组予以单侧开窗减压术治疗,比较两组疼痛程度、腰椎功能恢复情况及围手术期指标。结果两组术后1周、3个月VAS评分低于术前,且A组术后3个月VAS评分低于B组,差异有统计学意义(P<0.05);两组术后1周、3个月JOA评分高于术前,且A组术后3个月JOA评分高于B组,差异有统计学意义(P<0.05);A组术中失血量、手术时间、住院时间及费用优于B组,差异有统计学意义(P<0.05),而两组术后下床时间比较,差异无统计学意义(P>0.05)。结论腰椎间盘胶原酶溶解联合射频热凝术治疗青少年腰椎间盘突出症优于单侧开窗减压术治疗,具有术后恢复快、手术及住院时间短、术中出血量少、住院费用低等优点,但由于样本数量有限,仍有待进一步研究证实。Objective To compare the efficacy of radiofrequency thermocoagulation combined with low-dose collagenase chemolysis and unilateral fenestration discectomy in the treatment of adolescent lumbar disc herniation.Methods A total of 65 cases of adolescent lumbar disc herniation admitted to our hospital from June 2019 to June 2020 were selected and divided into group A(33 cases)and group B(32 cases)according to different treatment methods.Group A was treated with radiofrequency thermocoagulation combined with collagenase chemolysis,and group B was treated with unilateral fenestration decompression.The pain degree,lumbar function recovery and perioperative indexes were compared between the two groups.Results The VAS scores of the two groups at 1 week and 3 months after operation were lower than those before operation,and the VAS score of group A was lower than that of group B at 3 months after operation,the difference was statistically significant(P<0.05).The JOA scores of the two groups at 1 week and 3 months after operation were higher than those before operation,and the JOA score of group A was higher than that of group B at 3 months after operation,the difference was statistically significant(P<0.05).The intraoperative blood loss,operation time,hospitalization time and cost of group A were better than those of group B,and the difference was statistically significant(P<0.05),there was no significant difference in postoperative ambulation time between the two groups(P>0.05).Conclusion Lumbar disc collagenase dissolution combined with radiofrequency thermocoagulation is superior to unilateral fenestration decompression in the treatment of adolescent lumbar disc herniation.It has the advantages of rapid postoperative recovery,short operation and hospitalization time,less intraoperative blood loss and low hospitalization cost.However,due to the limited number of samples,further research is still needed to confirm.
