摘 要:目的分析纳米银聚氨酯敷料在骨科创面中的应用价值。方法选取2019年2月-2021年2月我院收治的86例骨科创伤患者,按照随机数字表法分为对照组和观察组,各43例。对照组采用聚氨酯敷料治疗,观察组采用纳米银聚氨酯敷料治疗,比较两组创面愈合效果、愈合时间、换药次数、VAS评分、血管内皮生长因子(VEGF)、血小板衍化生长因子(PDGF)、抗菌效果及凝血指标[活化部分凝血酶原时间(APTT)、凝血酶原时间(PT),凝血酶时间(TT)]。结果观察组创面愈合优良率高于对照组,差异有统计学意义(P<0.05);观察组创面愈合时间短于对照组,换药次数与VAS评分低于对照组,差异有统计学意义(P<0.05);两组VEGF、PDGF水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组菌落计数低于对照组,APTT、PT、TT长于对照组,差异有统计学意义(P<0.05)。结论纳米银聚氨酯敷料可优化骨科创面的愈合效果,缩短其愈合时间,同时减少换药次数、降低疼痛程度,提高创面生长因子水平,具有良好的抗菌、抗凝效果。Objective To study the application value of nano-silver polyurethane dressing in orthopedic wounds.Methods A total of 86 orthopedic wound patients admitted to our hospital from February 2019 to February 2021 were selected and divided into control group and observation group according to the random number table method,with 43 cases in each group.The control group was treated with a polyurethane dressing,and the observation group was treated with a nano-silver polyurethane dressing.The wound healing effect,healing time,dressing change times,VAS score,vascular endothelial growth factor(VEGF),platelet derived growth factor(PDGF),antibacterial effect and coagulation indexes[activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT)]were compared between the two groups.Results The excellent and good rate of wound healing in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The wound healing time of the observation group was shorter than that of the control group,and the number of dressing changes and VAS score were lower than those of the control group,the difference was statistically significant(P<0.05).The levels of VEGF and PDGF in the two groups were higher than those before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).The colony count in the observation group was lower than that in the control group,APTT,PT and TT were longer than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Nano-silver polyurethane dressing can optimize the healing effect of orthopedic wounds,shorten the healing time,reduce the number of dressing changes and the degree of pain,improve the level of wound growth factor,which has good antibacterial and anticoagulant effects.
