某基层医院2020年住院患者抗菌药物医嘱点评分析

摘　　要：目的分析笔者所在医院2020年住院患者抗菌药物医嘱点评情况。方法2019、2020年,每月抽取6个临床科室,重点抽查感染科、呼吸科、重症医学科、外科等科室,其余科室随机抽查,从抽查的每个科室随机抽取5份当月出院病历进行抗菌药物专项点评,每月抽取30份病历进行抗菌药物专项点评,并对全年点评结果进行总结、分析。结果2020年住院患者抗菌药物医嘱点评合理率为93.9%(338/360),较2019年的92.5%(333/360)有所升高;2020年下半年住院患者抗菌药物医嘱点评合理率较上半年有所升高。2020年住院患者抗菌药物医嘱点评不合理22份,其中其他用药不适宜和用法用量不适宜最多,均为36.4%;其次为用药途径不适宜(占13.6%)、遴选药品不适宜(占9.1%)、联合用药不适宜(占4.5%)。结论笔者所在医院抗菌药物合理使用水平较高,医疗机构应该加强抗菌药物的管理及应用,以促进抗菌药物的合理应用。Objective To summarize and analyze the comments on the inpatients'medical orders of antimicrobial drug in author's hospital in 2020.Methods In 2019 and 2020,6 clinical departments were selected every month,focusing on the infection department,respiratory department,icu,surgery department,and the rest of the departments were randomly selected.Five discharged medical records were randomly selected from each of the 6 departments,and a total of 30 medical records were reviewed according to the relevant rules and regulations.The review results of the whole year were summarized and analyzed.Results In 2020,the rational rate of inpatients'antibacterials orders was 93.9%(338/360),which was higher than 92.5%(333/360)in 2019,and the rational rate of inpatients'antibacterials orders in the second half of 2020 was higher than that in the first half of 2020.In 2020,there were 22 unreasonable comments of inpatients'antibacterials orders,of which the other inappropriate medical orders and the inappropriate usage and dosage were the most,both 36.4%;the second was inappropriate route of administration(13.6%),inappropriate selection of drugs(9.1%),and inappropriate combination of drugs(4.5%).Conclusion The author's hospital has a high level of rational use of antibiotics.Medical institutions should strictly manage and apply antibiotics to further promote the rational use of antibiotics.
