摘 要:目的探讨可溶性生长刺激表达基因2蛋白(sST2)联合B型钠尿肽(BNP)对心力衰竭(HF)患者诊断及预后的预测价值。方法检测确诊HF患者(试验组,62例)和非HF患者(对照组,58例)血清sST2和BNP水平,比较两组相关指标的差异。随访出院后12个月内主要心血管不良事件(MACE)发生情况。结果试验组sST2≥35 ng/mL患者所占比例为86.79%(46/53),高于对照组的13.21%(7/53)(P<0.01)。随访期间,试验组27例发生MACE,对照组5例。血清BNP预测HF患者预后的AUC为0.571,血清sST2预测HF患者预后的AUC为0.782,sST2联合BNP预测HF患者预后的AUC为0.850。结论 sST2可作为HF诊断及预后评估的特异性标志物,sST2与BNP联合检测对HF患者的诊断和预后预测有更大的应用价值。Objective To investigate the value of combined detection of serum soluble growth stimulation expression gene 2 protein(sST2) and B-type natriuretic peptide(BNP) in the diagnosis and predicting the prognosis of the patients with heart failure(HF).Methods Serum levels of sST2 and BNP were detected in 62 paitents with HF(group A) and 58 paitents without HF(group B).The differences of relevant data were compared between the two groups.The major adverse cardiovascular events were followed up for 12 months.Results The proportion of the patients with serum sST2 ≥ 35 ng/mL was 86.79%(46/53) in group A,which was higher than 13.21%(7/53) in group B(P<0.01).During follow up, 27 cases in group A and 5 cases in group B had major adverse cardiovascular events.The area under the ROC curve for predicing HF was 0.571 for BNP,0.782 for sST2,and 0.850 for combined use of BNP and sST2.Conclusion sST2 could be used as a specific biochemical indicator for the diagnosis and predicting the prognosis of the patients with heart failure, for which combined use of BNP and sST2 is better than BNP or sST2 alone.
